About this role

Job summary

To provide an expert role within the Integrated Diabetes Service for patients with Diabetes.

Assess, plan, implement and evaluate programmes of care for adults with diabetes, through hospital and home, developing individual care plans in partnership with the patient, their family and relevant others

Facilitate and monitor the management of diabetes patients across primary and secondary care, and identify other complex care areas requiring input

To support & educate Diabetes patients in a variety of settings.

To support staff caring for Patients with Diabetes in a variety of settings

To be an integral member of the Integrated Diabetes Service working with all members of the Multi-Disciplinary team.

Be a non-medical prescriber. Reflecting the requirements for prescribing principle areas, knowledge and competencies required to underpin the practice of prescribing.

The post holder will be experienced in starting and monitoring patients on CGM and HCL and in the interpretation of data.

Main duties of the job

Provide specialist diabetes care for adults with complex physical and mental health needs across hospital and community settings.

Develop and review care plans with patients, families and the multidisciplinary team.

Lead on diabetes management including interpreting clinical results and adjusting insulin/medications.

Deliver CGM/HCL support from assessment to device initiation and ongoing review.

Educate patients and carers on self-management, early warning signs and health promotion.

Work collaboratively in MDTs and liaise with health, social care and voluntary services.

Maintain care quality while managing own caseload and supporting junior staff.

Continue professional development and uphold required clinical registration.

Job description Job responsibilities

Assess, plan implement and evaluate programmes of care for adults with Diabetes and Mental Health conditions through hospital and home.

Develop individual Care plans in partnership with the patient, their family and relevant others including all members of allied health professionals involved in the patients care.

Act as a role model and clinical expert in Diabetes management.

Analyse signs and symptoms, laboratory tests, investigations and other measures in formulating a diagnosis.

Maintain high standards of care within own practice and monitor the standard of care delivered to all patients on caseload.

Contribute to the Multi-Professional Team, both internally and externally to provide an improved pathway for Diabetes management.

To attend MDT meetings as necessary to discuss patients diabetes management and develop care plans for complex

Provide support and education with appropriate referral and liaison to other professionals / agencies.

Undertake clinical procedures and interventions commensurate with the role, including changing and adjusting insulin doses and oral diabetic medications in accordance with national and local guidelines and practice.

Undertake CGM and HCL starts, including pre assessment, ordering of equipment and ongoing reviews of patients using these devices.

Work collaboratively with colleagues to actively seek out patients who will benefit from case management.

Work effectively with local health, social care, and housing and voluntary sector services

Engage patients and carers in individual health promotion actions.

Communicate with individuals, groups and communities to improve the health of the population

Work with patients and carers to teach and educate them about the early warning signs in order to facilitate rapid management of complications or crises

To provide cover where appropriate and work collaboratively with all members of the team to ensure adequate staffing levels

To actively support and maintain effective lines of communication between patients / carers and professionals

To manage own workload and support junior team members to prioritise workload safely and effectively.

Take responsibility for own professional development in agreement with line manager. Maintain current registration of the required qualification for the post

Person Specification

Education/Qualifications Essential

Degree in relevant subject or equivalent with HPC/NMC registration Post registration qualification in insulin management e.g. DAFNE educator Working knowledge of the NSF for Diabetes and relevant NICE guidelines

Desirable

Registered non-medical prescriber or willingness to achieve within the first 6 months in post

Corporate Essential

Ability to travel across sites and across Trust boundaries to attend meetings, etc.
