CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!We are seeking a Band 3 Rehabilitation Assistant to join our Hertsmere Community Team, based at Potters Bar Community Hospital. This is a part-time role (22.5 hours per week). A Wednesday to Friday pattern is preferred, with flexibility open to discussion.You will support the delivery of rehabilitation services to support independence. Working under the guidance of qualified therapists, you will:o Assist with personalised treatment programmeso Support patients with exerciseso Organise and maintain equipmento Provide feedback on patient progresso Support administrative tasks

You will be part of our holistic team committed to delivering quality care. The team includes Physiotherapists, Occupational Therapists, Nurses, Healthcare Assistants, Therapy Assistants, and administrators. We work closely with GPs, hospitals, social care providers, hospices, and other third-sector organisations, supporting patients with a wide range of conditions.We offer structured clinical supervision, training opportunities, and support for career progression within the Trust.

As this is a community-based role serving the entire Hertsmere area, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Main duties of the job

Key Duties in this role include:

To be an active member of the clinical rehabilitation team. To rehabilitate and maximise a patient's potential. To support and enable patients in a range of activities to promote independent living and monitor their wellbeing. Provide individual therapy treatment under the guidance of physiotherapy and occupational therapy colleagues. Manage a caseload of patients and maintain professional documentation. Build and maintain relationships with other local services to facilitate appropriate referrals and high-quality healthcare. Provide guidance and information to patients, health and social care professionals, carers and relatives and other non-professionals. To deliver appropriate treatment in a variety of settings including the patient's own home, residential and nursing homes. Actively participate in Continuing Professional Development and Clinical Supervision to support your own development and ensure clinical practice is maintained at a high standard.

Person Specification

Qualifications Essential

NVQ Level 3 or equivalent level of knowledge and experience. Good general education up to GCSE equivalent. Willingness to undertake training to meet competency framework.

Desirable

Care certificate or willingness to work towards Knowledge of OTAGO and other exercise programmes

Experience Essential

Experience of working in a healthcare environment or care environment Experience treating adult and/or elderly client groups

Desirable

Previous experience of working in the NHS. Experience in falls prevention and community work Knowledge of the role of registered professional OT and/or Physiotherapist. Use of adaptive equipment. Moving and handling experience

Skills Essential

Able to demonstrate commitment to high quality care and service provisions. Good communication skills. Competent use of English language (spoken and written). Good level of numeracy. Ability to record messages and information in an accurate and concise way. Work flexibly to accommodate patient/ service needs. Ability to work well within a team as well as individually. Well organised. Good observational skills. Ability to manage self and prioritise own workload. Basic computer skills.

Desirable

Ability to effectively use Systmone and Microsoft teams.
