Fixed term contract/ secondment opportunity for 10 months
We are seeking a person who enjoys working with people living with dementia and who values the importance of person-centered care, for a fixed term contract for 10 months. Secondments may be available for internal candidates with the agreement of your manager.
The Dementia Support Service is a well-respected, supportive and hardworking team of support workers, nurses and occupational therapists who are dedicated to optimising the care of in-patients with dementia and enabling best outcomes for them at the point of discharge.
As a dementia specialist practitioner your focus will be on providing specialist assessment and care planning for patients who present with complex needs, along with interventions to enable engagement in care. You will promote person centered approaches, working collaboratively with patients, their families/carers and staff.

Main duties of the job
On a daily basis you will see in-patients referred to the team across different specialties in the hospital and will be expected to prioritise your caseload as well as that of the Dementia Support Workers. You will undertake specialist dementia needs assessments and care planning to enable a unified, person-centered approach to optimize care, working alongside families/carers and staff.
This role requires a creative approach to problem solving, and the proactive provision of interventions and reasonable adjustments to maintain skills, engagement in care and best outcomes.
You will be working independently as well as working alongside the dementia support team and many teams. A thorough induction and regular supervision will be provided.
This post is fixed term for 10 months, 18 hours per week

Job description Job responsibilities
To manage a specified case load of patients, providing information and support, to enable patients to make informed choices about their care and understand the rational of investigations and treatments. To work closely with the Consultants, medical and nursing staff, multi-disciplinary team and Heads of Department within the Directorate. To ensure that patients are seen within recommended time scales and targets where these are in place. To visit inpatients as appropriate to ensure continuity of care and to give advice/ support. To accept referrals from medical/nursing staff both within and outside the Trust, of newly diagnosed and existing patients To periodically review caseload and to discharge patients appropriately within divisional guidelines To respond to telephone enquiries and co-ordinate admission or changes in appointment dates as warranted by the patients clinical condition, including operation of a telephone helpline for the relevant speciality if required. To undertake a comprehensive assessment of the patients symptoms and in liaison with the Medical team organise investigations as required. To supply and/or administer medication according to Patient Group Directions or in accordance with the Trusts Prescribing Policy for registered nurse prescribers. To monitor the effectiveness and efficacy of treatments/drug therapy and implement any changes necessary.
