Job summary
Our NLFT London Prison situated in North London, has a vacancy for a Early Days in Custody Senior Practitioner. The core of this role is to support the delivery of care, which is currently in the form of New Models of Care.
The ideal candidate will need to ensure they are operating in line with the New Models of care by providing a high standard to individuals within our prison settings who are presenting with mental health problems.
The post holder will work with staff across healthcare and the wider prison establishment, providing specialist advice particularly to GPs in the assessment and treatment of patients on the prison residential wings.
The successful candidate will need to adapt to and embrace a recovery and enablement model to seamlessly realign prisoners with mental health problems alongside their local community- based mental health service upon release from prison.
The post holderwill be a qualified RMN with sound experience in Mental Health, knowledge and experience of forensic mental health service provision and offender care.

Main duties of the job
The post holder will work with the team lead and therapies lead to ensure the team work to an agreed operational policy, offering a high standard and integrated triage, assessment and intervention for individuals within our prison settings who are presenting with mental health problems in line with the New Models of Care.The post holder will ensure that all patients are seen, receive a mental health triage, risk assessment and safety plan and to establish any immediate and ongoing health needs. As an Early Days in Custody Senior Practitioner, you will be innovative, bringing fresh ideas and solutions to the challenges of providing healthcare in a prison. You will be responsible for identifying those with mental health concerns including those in mental health crises and referring them on to the appropriate team.

The post holder will be required to support the delivery of care currently in the form of New Models of Care, enabling the service to meet the London Wide service specification standards for Prison Healthcare.
The post holder will be required to carry out triage and comprehensive assessments and be able to produce effective care plans that are recovery focused and seek to minimise risk. The post holder will have an excellent understanding of risk assessment and management and be able to share this knowledge with colleagues and others involved in the patients care in order to minimise risk.
The post holder will manage a caseload of patients and be able to prioritise and coordinate their care according to their needs and ensure that there is continuity of care throughout the period of detention.
The post holder will provide support, consultation, and formal and informal training to colleagues and other staff. Excellent communication and relationship management skills are essential to this post.
Person Specification
Education and Qualifications Essential
Dip/BSc (Hons) in Mental Health Nursing, Social Work or Occupational Therapy Evidence of further post-registration education and training or equivalent teaching and assessing qualification.
Desirable
ENB 998 or equivalent teaching and assessing qualification
Previous Experience Essential
Post registration experience as a registered professional (RMN, OT, SW) preferably within a Forensic Setting. Experience in a leadership role

Experience of working within a Trauma informed framework
Skills and Abilities Essential
Ability to plan and organise a range of complex clinical activities which require judgments to be made on competing factors. Ability to undertake risk assessments using standardised or semi-structured risk assessment methods. Ability to network, collaborate and liaise with other agencies. Ability to work independently and autonomously.
Desirable
Ability to contribute to new strategies and policy development. Ability and confidence to take responsibility in planning, implementing, monitoring, and evaluating the standards of care programmes in the community.
