Job summary
An opportunity has arisen for an enthusiastic and motivated nurse to join our Integrated Sexual Health Services team as a Young Persons Contraceptive Nurse. The postholder will contribute to the delivery of highquality, patientcentred sexual health and contraceptive care across Sunderland.
Our service provides comprehensive sexual health provision, including screening and treatment of sexually transmitted infections (including HIV), full contraceptive services, and support to the Termination of Pregnancy service. We are committed to delivering safe, accessible, and inclusive care within a friendly and supportive multidisciplinary team. The successful candidate will divide their time between communitybased young peoples outreach and clinical sessions within the main Sexual Health Clinic.
You will Provide contraception, sexual health screening, and health promotion to vulnerable young people. Deliver care in a range of outreach settings including schools, colleges, community venues, and young peoples homes. The post holder must demonstrate a strong understanding of safeguarding principles and work collaboratively with the wider multidisciplinary team to support young people at risk.
The role involves a significant amount of travel during working time and the successful applicant must be able to meet the travel requirements of the post. The Trust is committed to making reasonable adjustments for applicants in line with the Equality Act 2010.
Main duties of the job
1. Providing Contraceptive Care
Assessing young peoples contraceptive needs in a sensitive, non judgemental way and issuing chosen contraception via PGD. This will include emergency contraception and LARCs.
2. Sexual Health Screening and Treatment
Carrying out STI screening, assisting with accessing treatment and partner notification.
Providing clear, age appropriate health education about STIs, prevention, and safer sex
3. Outreach Work with Vulnerable Young People
This is a distinctive part of the role to reach young people who may not access clinic based services
4. Safeguarding and Risk Assessment
The role requires the ability to recognise signs of exploitation, abuse, coercion, or neglect, and to complete robust safeguarding assessments while working collaboratively with other services.

6. Working Within a Multidisciplinary Team

The specialist nurse will:
Work as part of the multidisciplinary team working under the direction of line manager/Matron.
Assess care needs.
Develop programmes of care and implement and evaluate these programmes of care for young people accessing Contraceptive Services.
The focus of the post holders role will be clinical with specialist young persons clinics, outreach visits, clinical review and administration relating to young people and child protection and core sexual health service provision.
The post holder will actively try and engage young people who are unwilling/unable to attend mainstream services.
Undertake all relevant forms of care without direct supervision and take charge in the absence of the person with continuing responsibility.
Be expected to manage extremely sensitive situations
Identify any child safeguarding issues and adhere to the Trust safeguarding policies and procedures.
Identify those young people who are at high risk of pregnancy and provide support and education with regards to contraception and safer sex.
The post holder will see clients on an individual basis, assess, plan, implement and evaluate a programme of client care which is evidence-based utilising all available resources and taking into consideration lifestyle, race, gender, family and cultural background and ensure involvement, as appropriate with the client, family carers and significant others. They will demonstrate competence and sensitivity in undertaking a detailed sexual and social history to establish the needs of the individual client and support, counsel or refer for advice as appropriate.
Provide nurse led outreach sexual health clinics to young people in secure units, seeing clients who are often the most difficult and complex to engage in maintaining good sexual health.
Support the development of nursing practice within the specialist field through the use of evidence-based practice and in line with agreed national standards and contribute to the body of nursing knowledge.
Provide safeguarding, young persons governance and follow up by means of a virtual diary for all young people where there are any concerns. These concerns come from counselling, nursing and medical staff throughout the service.
Work as an autonomous practitioner within their sphere of practice.
Continuously improve clinical standards in line with clinical governance framework.
Exercise accountability as set out in the NMC Code.
Contribute to training and education within the specialist area.
Provide specialist clinical advice and support to patients and clinical staff within the Trust on the management and care of patients within the specialist area.
Provide direct access for patients, either face to face or telephone consultation Represent the service nationwide in the absence of the line manager/Matron.
Play a key role in ensuring that national and local standards/targets for the specialist area are met.
