Job summary

Woodgrange Medical Practice is looking for a part time Advanced Nurse Practitioner.

We are a well organised, efficient Practice, with a list size of 16000 and growing. We are based in a spacious converted bank, which has had several extensions to provide a comfortable and modern clinical environment. There are excellent transport links and we are close to the Olympic Park and Westfield shopping Centre.

We have a strong ethos of teaching and training and you will be welcomed into our large Primary Care team of 13 Doctors, GPSi in Dermatology/surgery, 2 Advanced Nurse Practitioners, 2 Practice Nurses , 3 HCAs, Diabetic Nurse Specialist, 3 Pharmacists and you will instantly become a valued member of the team.

Every clinical and administrative member of staff is encouraged to develop professionally. The doctors have been supported and encouraged to specialise; each has their own interest e.g. Substance Misuse, Mental Health, Dermatology, Minor surgery, Paediatrics and Womens health.

Job description Job responsibilities

The post holder will deliver high-quality, patient-centred care while working collaboratively with multidisciplinary teams to improve health outcomes and ensure safe, effective service delivery.

The ANP will utilise advanced clinical knowledge and decision-making skills to assess patients presenting with acute, chronic, and complex conditions. They will independently manage episodes of care, prescribe medications where qualified, order and interpret diagnostic investigations, and make appropriate referrals to specialist services when required.

In addition to direct patient care, the ANP will provide clinical leadership, support service development, contribute to quality improvement initiatives, and act as a mentor and role model to colleagues across the organisation.

Clinical Practice

Undertake comprehensive clinical assessments, including detailed history taking, physical examination, and risk assessment.

Diagnose and manage a wide range of acute, chronic, and complex health conditions.

Formulate differential diagnoses and develop evidence-based treatment plans.

Independently prescribe and review medications within professional competence and prescribing authority.

Order, interpret, and evaluate diagnostic investigations including laboratory tests, imaging, and other relevant assessments.

Provide timely interventions, treatment, and follow-up care to achieve positive patient outcomes.

Recognise deteriorating patients and initiate appropriate escalation and emergency responses.

Maintain accurate and contemporaneous clinical records in line with professional and organisational standards.

Patient Care and Communication

Deliver compassionate, person-centred care that respects patient dignity, preferences, and cultural needs.

Engage patients, families, and carers in shared decision-making and care planning.

Provide health education and advice to support informed choices and self-management.

Communicate effectively with patients, relatives, and healthcare professionals across all levels.

Promote health and wellbeing through preventative healthcare and health promotion activities.

Leadership and Professional Practice

Provide clinical leadership and expert advice to nursing and multidisciplinary colleagues.

Act as a role model demonstrating professional excellence and high standards of clinical practice.

Support workforce development through mentoring, supervision, and clinical teaching.

Contribute to the development and implementation of clinical policies, procedures, and care pathways.

Promote evidence-based practice and support innovation within the service.

Multidisciplinary Working

Work collaboratively with medical staff, nurses, allied health professionals, social care teams, and external agencies.

Participate in multidisciplinary team meetings and contribute to complex case discussions.

Coordinate patient care across organisational and professional boundaries to ensure continuity and quality of care.

Develop effective working relationships that support integrated service delivery.

Quality, Governance and Service Improvement

Participate in clinical audits, service evaluations, and quality improvement initiatives.

Contribute to the monitoring and improvement of patient safety and clinical effectiveness.

Ensure compliance with professional standards, legislation, and organisational policies.

Report and manage incidents, complaints, and risks in accordance with governance procedures.

Support the implementation of best practice guidelines and quality standards.

Education and Development

Maintain and develop advanced clinical competence through continuing professional development.

Participate in mandatory training and professional learning activities.

Support the education and development of students, junior staff, and colleagues.

Share specialist knowledge and promote a culture of learning and professional growth.

Safeguarding and Risk Management

Identify and respond appropriately to safeguarding concerns involving children and vulnerable adults.

Person Specification

Qualifications Essential

Essential Qualifications Registered Nurse with current registration with the relevant professional regulatory body. Master's degree in Advanced Clinical Practice or equivalent advanced practice qualification. Independent/Supplementary Prescribing qualification. Evidence of ongoing professional development. Essential Experience Significant post-registration clinical experience. Experience working at an advanced practice level. Experience of autonomous clinical decision-making and patient management. Experience of prescribing and managing complex patient presentations. Experience working within multidisciplinary teams. Essential Knowledge and Skills Advanced assessment, diagnostic, and clinical examination skills. Excellent communication and interpersonal abilities. Strong leadership and mentoring skills. Ability to prioritise workload and manage competing demands. Sound understanding of clinical governance, safeguarding, and risk management. Knowledge of current healthcare legislation, policies, and evidence-based practice.

Desirable

Eligible to work in General Practice in the NHS Desirable Criteria Experience in primary care, urgent care, community services, or specialist clinical settings. Non-medical prescribing experience in a relevant clinical area. Experience leading service improvement or quality initiatives. Teaching, supervision, or mentorship qualifications. Participation in research, audit, or service development projects.
