Empath Trustbridge Hospice is seeking a compassionate and dedicated RN Case Manager to join our team serving patients and families across South Palm Beach County. In this role, you’ll have the unique opportunity to provide comfort, dignity, and support to individuals facing advanced illness—while working alongside a collaborative, mission-driven team. Your schedule would be: 8:30am - 5pm & every other weekend Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness.
