In the challenging yet rewarding role of a Case Manager, you will facilitate a strategic planning method called the Child and Family Team process. This process supports youths with complex behavioral or emotional needs in achieving their optimum level of wellness through the development of individualized service plans, linkage to community resources, partnership with family members and others involved in their lives, and the coordination of behavioral and physical health care.
