About this role
AMM is hiring a CMA or RMA to work as our Patient Care Coordinator for Care Management (CM). The PCC is responsible for coordinating a range of activities/tasks with patients. They will work within the scope of practice, and in concert with the primary care Provider, patients, caregivers, family members, and other members of the Care Management team. Local travel may be required. All CM employees must work on campus in Jacksonville for their first 90 days.
