Your Role: Our Clinical Homecare Nursing team really love what they do! Working in Partnership with our NHS colleagues to provide individualised quality nursing care to patients requiring Parenteral Nutrition and other IV therapies in their home. Key Details: Location: Working with trusts and patients in South Wales and the surrounding areas. Working hours: Monday to Sunday, 40 hours over 5 days per week on a rolling rota. The shifts are structured as split shifts to accommodate patient needs. This means you will typically work around 4 hours in the morning to disconnect patients, and another 4 hours in the evening to reconnect them, with a break in the middle of the day. Flexibility is essential; some days may be longer or shorter depending on patient visits, but overall, average out to 40 hours per week. The salary for the role is £43,391 per annum. associated care and support to patients who are unable to be trained to independence.</li> <li patients/carers with regards to the equipment and infusion devices required for infusion/administration of therapies to independence.</li> <li patients/carers in the connection and disconnection of PN and IV therapies to independence.</li> <li patients/carers in the care and management of their CVAD in accordance with local policies and national guidelines to independence.</li> <li autonomous decisions to resolve out of hours queries and escalate to the Senior on-call if required.</li> <li Regional Lead Nurse in discharge management including pre-discharge assessment visits and home assessments and following relevant processes, to provide a seamless service.</li> <li patients – complete all relevant documentation and escalate any issues.</li> </ul> Experience Required: <ul style="margin-bottom:0.0cm;margin-top:0.0px" type="disc"> <li style="line-height:normal;background-color:white;margin-top:0.0cm;margin-right:0.0cm;margin-bottom:8.0pt;font-size:11.0pt;font-family:Calibri, sans-serif">Intensive Care / High dependency / General surgery/ nursing care experience at a level equal to top of Band 5.</li> <li style="line-height:normal;background-color:white;margin-top:0.0cm;margin-right:0.0cm;margin-bottom:8.0pt;font-size:11.0pt;font-family:Calibri, sans-serif">Current clinical experience minimum of last 2 years.</li> <li style="line-height:normal;background-color:white;margin-top:0.0cm;margin-right:0.0cm;margin-bottom:8.0pt;font-size:11.0pt;font-family:Calibri, sans-serif">Competent in the management and delivery of PN and other IV Therapies.</li> <li style="line-height:normal;background-color:white;margin-top:0.0cm;margin-right:0.0cm;margin-bottom:8.0pt;font-size:11.0pt;font-family:Calibri, sans-serif">Competent in the care and management of CVADs.</li> <li style="line-height:normal;background-color:white;margin-top:0.0cm;margin-right:0.0cm;margin-bottom:8.0pt;font-size:11.0pt;font-family:Calibri, sans-serif">Blood samples from CVADs.</li> </ul> What you need to succeed: <ul style="margin-bottom:0.0cm;margin-top:0.0px" type="disc"> <li Certificate or equivalent (or attain within 12 months).</li> <li management skills and the ability to work well against strict timescales.</li> <li
