Main duties of the job At IC24, we are seeking an experienced Practice Nurse to become part of our Primary Care team at Allied Medical Practice. The role is rewarding, requiring a full range of nursing skills to provide high-quality, patient-centered care within a multidisciplinary team. Practice nurses will work in a busy primary care environment, delivering chronic disease management, immunisations, health screening, and minor treatments. Prevention, early intervention, and health promotion are crucial parts of the role. This involves working within an integrated healthcare system to improve access and reduce secondary care service pressure, ensuring patients receive appropriate care in a timely manner.

What youll focus on

Delivering high-quality nursing care

Youll provide assessment, treatment, screening, health education services and advice to patients, working in accordance with the NMC Code of Conduct and practice protocols and guidelines.

Managing long-term conditions

Youll diagnose, monitor and support the development of individual management plans for patients with conditions such as diabetes, respiratory disease and cardiovascular disease, working in partnership with patients and other healthcare professionals.

Prevention, screening and health promotion

Youll undertake NHS Health Checks, cervical screening, immunisations, travel health advice, smoking cessation support and wider preventative health interventions.

Clinical procedures and treatment delivery

Youll carry out wound care, ear care, phlebotomy, ECGs, blood pressure monitoring and other routine clinical procedures safely and in line with practice policy.

Supporting quality and clinical governance

Youll ensure accurate coding and recording of patient data to support QOF and Enhanced Services and contribute to high standards of clinical governance and best practice.

Working within a multidisciplinary team

Youll work both independently and in participation with GPs, HCAs, General Practice Assistants and wider clinical colleagues to deliver safe, effective and coordinated patient care.

Risk assessment and safeguarding

Youll recognise safeguarding concerns and wider risk issues (including domestic abuse, child protection and vulnerable adults), ensuring appropriate escalation in line with policy.

Is a Registered Nurse with current NMC registration Has experience working in general practice or primary care Has experience managing long-term conditions Demonstrates strong clinical judgement and safe decision-making Communicates clearly and effectively with patients and colleagues Is confident working independently and as part of a team Has strong organisational skills and attention to detail in clinical documentation Is committed to delivering compassionate, patient-centred care

Person Specification

Qualifications Essential

Registered Nurse with current NMC registration, experience in general practice or primary care, experience managing long-term conditions, strong clinical judgement and decision-making skills, effective communication with patients and colleagues, capability to work independently and within a team, strong organizational skills, and a commitment to compassionate care.
