About the Role
We are seeking a highly motivated and experienced Lead Practice Nurse to join our dynamic and supportive primary care team. This is an exciting leadership opportunity for a dedicated nurse who is passionate about delivering excellent patient care and driving service improvement. As Lead Nurse, you will provide clinical leadership and operational oversight to the nursing team, ensuring the delivery of safe, effective, and patient-centred care. You will play a key role in shaping services, improving patient outcomes, and supporting the ongoing development of nursing staff.
Main duties of the job
Clinical Leadership
Act as a strong role model, providing visible and supportive leadership. Deliver high-quality, evidence-based clinical care. Maintain advanced clinical skills and actively contribute to patient care.
Team Management
Line manage Practice Nurses and HCA's. Oversee staff rotas to ensure appropriate skill mix. Conduct appraisals and performance reviews. Support staff development and training.
Infection Prevention & Control (IPC)
Act as the IPC Lead. Ensure compliance with national and local IPC guidelines. Lead audits and implement improvements. Maintain a safe and clean clinical environment.
Long-Term Conditions & Diabetes Care
Lead on the management of long-term conditions, with a focus on diabetes. Ensure effective and high-quality chronic disease clinics. Support the achievement of QOF and other performance targets.
Vaccination Programmes
Lead the delivery of seasonal vaccination programmes including flu and COVID-19. Ensure safe storage, handling, and administration of vaccines. Coordinate clinics and staffing to meet patient demand.
Capacity & Demand Management
Monitor and manage appointment availability Improve patient access through effective clinic planning. Align staffing and service provision to meet demand.
Quality, Governance & Compliance
Ensure services meet CQC standards and regulatory requirements. Lead clinical audits and quality improvement initiatives. Maintain up-to-date policies and procedures.

Person Specification
Qualifications Essential
Registered Nurse (NMC registered) Evidence of continued professional development
Desirable
Nurse Prescriber qualification Experience leading vaccination programmes (e.g. flu/COVID clinics) Experience working towards QOF targets Experience in quality improvement or audit work Leadership/management qualification
Experience Essential
Experience in primary care or community nursing Proven experience in long-term condition management (including diabetes) Previous leadership or supervisory experience
Desirable
