Main duties of the job

The Practice Nurse will deliver high-quality nursing care to patients within the GP practice, supporting a wide range of clinical services including chronic disease management, health promotion, immunisations, cervical screening, wound care, and routine nursing treatments. The role involves caring for patients with long-term conditions such as diabetes, asthma, COPD, and hypertension, while encouraging healthy lifestyle choices and preventative healthcare.

Responsibilities also include maintaining accurate and up-to-date clinical records, contributing to quality improvement initiatives, and ensuring adherence to infection prevention, control, and safeguarding policies and procedures.

Job SummaryThe Practice Nurse will provide high-quality nursing care to patients within the GP practice, supporting the delivery of safe, effective, and patient-centred services. The role includes chronic disease management, health promotion, immunisations, cervical screening, wound care, and general treatment room duties.

The successful candidate will work collaboratively with the multidisciplinary team to improve patient outcomes, support preventative healthcare initiatives, and contribute to the ongoing development of practice services.

Main Duties and ResponsibilitiesClinical ResponsibilitiesDeliver routine nursing care and treatment room services to patients.Administer childhood and adult immunisations and vaccinations in accordance with national guidelines.Undertake cervical cytology screening.Provide wound care, dressings, ear irrigation, and suture removal.Carry out health assessments and NHS Health Checks.Support the management of long-term conditions, including:DiabetesAsthmaCOPDHypertensionUndertake chronic disease monitoring and annual patient reviews.Provide travel health advice and vaccinations where appropriate.Perform clinical procedures including:Blood pressure monitoringECGsSpirometryPhlebotomyAdminister prescribed medications and injections safely and effectively.Maintain accurate, contemporaneous, and confidential patient records using the clinical system.Health PromotionPromote healthy lifestyle choices and preventative healthcare.Provide advice and support relating to smoking cessation, healthy eating, exercise, alcohol reduction, and weight management.Encourage and support patients in the self-management of long-term conditions.Participate in health promotion campaigns, screening programmes, and vaccination initiatives.Infection Prevention and SafeguardingAdhere to infection prevention and control policies and procedures.Ensure the safe handling and disposal of sharps and clinical waste.Maintain cleanliness, organisation, and stock control within clinical areas.Recognise, escalate, and report safeguarding concerns in line with local and national guidance.Work in accordance with practice policies, clinical governance requirements, and NHS standards.Administrative ResponsibilitiesMaintain accurate clinical documentation and coding.Participate in clinical audits, quality improvement projects, and QOF-related activities.Attend practice meetings, mandatory training sessions, and continuing professional development activities.Support the efficient delivery and ongoing development of nursing services within the practice.

Person Specification

Qualifications Essential

Registered Nurse with current Nursing and Midwifery Council registration. Experience in primary care, community nursing, or general practice nursing. Knowledge of chronic disease management and immunisation programmes. Excellent communication and organisational skills. Ability to work independently and as part of a multidisciplinary team. Cervical cytology qualification.
