Job summary <p style="margin:0px;font-size:inherit;padding:5px 0px;">We have an exciting opportunity for a <strong style="font-weight:700;">Band 4 Associate Practitioner</strong> to join our dedicated team on <strong style="font-weight:700;">Sapphire Ward</strong> at Gravesham Community Hospital.</p><p style="margin:0px;font-size:inherit;padding:5px 0px;">Working under the supervision of a Registered Nurse, you will play a key role in delivering safe, high-quality care to patients in a rehabilitation setting, following national and local priorities, Trust policies, and Nursing & Midwifery Council standards. <strong style="font-weight:700;">This role requires NMC registration</strong> and is ideal for someone with health or social care experience who is ready to develop their clinical skills, work as part of a supportive multi-disciplinary team, and make a genuine difference to patient outcomes.</p>

Main duties of the job <p style="margin:0px;font-size:inherit;padding:5px 0px;">As a Band 4 Associate Practitioner you will be:</p><ul style="margin-top:0px;margin-bottom:10px;"><li style="font-size:inherit;"><p style="margin:0px;font-size:inherit;padding:5px 0px;display:unset;">Deliver patient care in accordance with Trust policies, procedures, and professional standards as delegated by the Nurse in charge</p></li><li style="font-size:inherit;"><p style="margin:0px;font-size:inherit;padding:5px 0px;display:unset;">Undertake technical nursing tasks such as dressings, glucose monitoring, and other delegated clinical procedures</p></li><li style="font-size:inherit;"><p style="margin:0px;font-size:inherit;padding:5px 0px;display:unset;">Assist with patient assessments and implement care plans set by the Registered Nurse</p></li><li style="font-size:inherit;"><p style="margin:0px;font-size:inherit;padding:5px 0px;display:unset;">Accurately document all care provided using EMIS and ensure confidentiality is maintained at all times</p></li><li style="font-size:inherit;"><p style="margin:0px;font-size:inherit;padding:5px 0px;display:unset;">Recognise and report changes in patient condition promptly to the Nurse in charge</p></li><li style="font
