The purpose of the post is:

o To carry out skilled interventions as delegated by the qualified Physiotherapist or Band 4 Support worker. This could be in the patient's home, or in the community in a public building. This role is to work alongside a physiotherapist and Band 4 support worker to deliver pulmonary rehabilitation.

o To monitor the patient's response to therapy intervention and report back on the outcome.

o To encourage patients and carers in an active approach to increase their exercise tolerance and confidence to live a more active life.

o To promote exercise and chronic respiratory disease management.

Main duties of the job

Clinical Activities

o Understanding the concepts of pulmonary rehabilitation.

o Good verbal and written communication skills.

o Commitment and understanding of team working.

o To monitor patients' responses to therapy intervention(s) and communicate this to therapist.

o To plan and prioritise own casework and organise time effectively.

o To plan and implement therapy interventions (individually and in groups), acting within competencies.

o To motivate patients to engage in pulmonary rehabilitation.

o To advise and educate patients and carers, within competencies, e.g. safe use of community equipment.

o To visit patients' homes to assess (under the guidance of a qualified therapist) and follow a guided exercise plan.

o To be responsible for patients allocated on a case-by-case basis, working without direct supervision, reporting back progress and informing the therapist of any problems.

o To operate as a lone worker at times, with some opportunity for contact with qualified staff during the day.

Person Specification

Personal Qualities Essential

Willing to actively participate in, and record, CPD activities and in service training.

Qualifications Essential

Experience of working with Respiratory/chronic conditions Good general education (to GCSE or equivalent). NVQ level III qualification in therapy/care or equivalent relevant qualification. Previous work in the community or hospital setting.

Desirable

Experience of working with vulnerable adults. Experience of working using a therapeutic approach. Experience as a support worker working in health or a related area Experience of working as a lone worker.
