Job summary

ARE YOU THE PERSON WE ARE LOOKING FOR?

We are seeking a highly motivated and confident adult trained Registered Nurse, Physiotherapist or Occupational Therapist to work within our Flow Team as part of the LLR discharge hub.

You will be supporting 13 community hospital wards in the allocation, safe transfer and discharge of all patients coming into and leaving the wards. We are looking for people who have a keen interest in transfer of care, capacity, flow and discharge planning.

The right candidates will need to have a moderate level of IT skills, basic data collection abilities, excellent negotiating skills and show a holistic approach and a passion for discharge and transfer of care. You will work as part of our team but there will be times when you will be working autonomously.

You will also have the opportunity to provide support and education to our ward teams.

We currently have an agile working environment but also divide our working time between the 8 sites within Community Health Services, so the ability to drive and access to a vehicle that can be used for work purposes is essential.

Main duties of the job

Allocating patients to LPT wards from external services, liaising with acute and out of area hospitals and LPT Community Hospital (CoHo) wards.Liaising with system partners and building relationships to provide successful integrated care systems.Supporting LPT CoHo wards in the facilitation of safe and timely discharges, often supporting in complex cases, working with teams closely to provide resolutions and support in difficult and complex discharge situations.To collate and submit data daily and weekly as part of the team to ensure that local and national standards are maintained and improvements are made where needed.Triaging patients that are referred into the hub.Working with your buddy ward to enable timely discharge and flow.
