About The RoleAs a Nurse Practitioner, you will provide autonomous clinical care to patients across a broad range of acute and chronic conditions. You will assess, diagnose, treat, prescribe, and refer patients as appropriate, helping to improve access to healthcare services within the practice.

Main duties of the job
Key Responsibilities
Conduct comprehensive patient assessments and clinical examinations. Diagnose and manage acute and long-term health conditions. Independently prescribe medications within your scope of practice. Order, interpret, and act upon diagnostic tests and investigations. Provide health promotion, disease prevention, and lifestyle advice. Refer patients to secondary care and specialist services when required. Maintain accurate and timely patient records. Work collaboratively with GPs, practice nurses, pharmacists, and other healthcare professionals. Participate in clinical governance, audit, and quality improvement activities. Essential Requirements
Registered Nurse with current registration with the relevant professional body. Qualified Nurse Practitioner with advanced clinical assessment skills. Independent Prescriber qualification. Experience working in primary care or general practice. Excellent communication, organisational, and decision-making skills. Ability to work autonomously and as part of a multidisciplinary team. Desirable
Experience managing chronic disease clinics. Knowledge of UK primary care systems and pathways. Experience using electronic patient record systems we use System One.
About us
Hirsel Medical Centre is a small and friendly GP practice based in North Ormesby Health Village with approximately 4000 patients. We have 2 GP partners, 1 Practice Nurse and 1 HCA along with a small administration and reception team.
