Job summary
To provide specialist assessment, treatment, management, and education relating to wound care and tissue viability within the general practice population. The post holder will work collaboratively with GPs, practice nurses, community services, and multidisciplinary teams to prevent and manage wounds, promote healing, reduce complications, and improve patient outcomes.
Main Duties
Assess, plan, implement, and evaluate care for patients with acute and chronic wounds.Perform comprehensive wound assessments and develop individualised treatment plans.Manage a range of wounds including pressure ulcers, leg ulcers, diabetic foot ulcers, surgical wounds, skin tears, and traumatic wounds.Provide evidence-based wound care, including dressing selection and application of advanced wound management techniques.Assess patients for risk of pressure damage and implement preventative measures.
Main duties of the job
Monitor wound healing progress and review treatment plans accordingly.Recognise and manage wound infections, escalating concerns and referring to appropriate services when required.Deliver compression therapy for eligible patients with venous leg ulcers and oedema.Educate patients, carers, and families on wound care, skin integrity, pressure ulcer prevention, nutrition, and self-management strategies.Provide specialist tissue viability advice and support to GPs, nurses, healthcare assistants, and other members of the multidisciplinary team.Liaise with community nursing teams, podiatry services, specialist tissue viability teams, and secondary care providers to ensure continuity of care.Maintain accurate and contemporaneous patient records using electronic clinical systems.Participate in clinical audits, quality improvement initiatives, and service development related to wound care and tissue viability.Ensure practice complies with local and national wound care guidelines, policies, and best practice standards.Support the training, supervision, and competency development of clinical staff in wound management and tissue viability.Adhere to infection prevention and control procedures, safeguarding responsibilities, and professional standards set by the Nursing and Midwifery Council.Maintain up-to-date knowledge of developments in wound care and tissue viability through continuing professional development.

Clinical Responsibilities
Assess, plan, implement, and evaluate care for patients with acute and chronic wounds.
Undertake comprehensive wound assessments using evidence-based practice.
Develop and implement individualised wound management plans.
Manage a range of wound types, including:

Perform wound dressings and advanced wound care interventions.

Lead on pressure ulcer prevention initiatives within the practice.
Promote evidence-based wound care practices.
Support the implementation of local and national wound care guidelines.
Participate in wound care audits and quality improvement projects.
Monitor and evaluate wound care outcomes.
Maintain knowledge of current wound care products and technologies.

Report incidents, safeguarding concerns, and adverse events according to policy.
Maintain professional registration and comply with the requirements of the Nursing and Midwifery Council.

Person Specification
Skills & Knowledge Essential
Advanced knowledge of wound healing and tissue viability principles Understanding of pressure ulcer prevention and management Knowledge of compression therapy and leg ulcer management. Excellent clinical assessment skills. Strong communication and patient education skills. Ability to work autonomously and prioritise workload effectively. Competence in clinical documentation and use of electronic patient records.
Qualifications Essential
Registered Nurse (Adult) with current registration with the Nursing and Midwifery Council. Post-registration qualification in wound care, tissue viability, or equivalent specialist training Evidence of continuing professional development. Independent Nurse Prescriber qualification
