We are looking for a motivated Senior Therapy Assistant to join our friendly SDEC and ED teams, delivering therapy services right at the front door. You'll play a pivotal role in supporting same day discharges, helping patients safely return home, and reducing unnecessary admissions.
Your role will involve assisting with patient assessments, focusing on mobility, transfers, and daily activities. You'll work closely with our multidisciplinary team to develop personalised discharge plans and ensure continued support from Community services, making every patient's journey as smooth as possible.
This position covers a flexible 7-day service from 8am to 6pm, offering you the chance to be part of a supportive team dedicated to excellent patient care.
Main duties of the job
As part of the team, you will assist therapists with their patient caseload, as well as help with essential administrative and housekeeping duties, keeping our service running efficiently and making a positive impact every day. You will apply your organisational and clinical skills to manage work and respond to service needs. Using your clinical expertise and strong communication, you will adjust care as necessary and refer patients to appropriate community services.
About us The Trust's services are provided from three locations: o The Countess of Chester Hospital: providing 473 general and acute beds. o Ellesmere Port Hospital: providing 60 beds as a rehabilitation, intermediate and outpatient facility. o Tarporley War Memorial Hospital: a base for community services which serve the local rural population. NVQ level 4 or equivalent ECDL or CLIAT qualification
Experience Essential
Significant experience working in a health or social care setting, working as a therapy assistant Experience of delivering therapy programmes to patients with complex needs Supervisory experience Knowledge of a range of presenting conditions. Carry out interventions using a criteria led discharge protocol Able to undertake reviews and amend action plans accordingly
