About this role
Job summary
You will deliver skilled, holistic nursing care to patients with life-limiting illness, working autonomously and in partnership with GPs, District Nurses and the wider multidisciplinary team. You will also support families, contribute to service coordination, and help ensure a responsive, compassionate service.
Main duties of the job
Provide compassionate end-of-life nursing care in patients homes Assess, plan and deliver individualised care, including symptom management Administer medications and manage clinical interventions (e.g. syringe drivers, catheter care).
Offer emotional support and guidance to patients and carers. Provide Cover and Support for Wellbeing Services.
Work collaboratively with healthcare professionals to provide seamless care.
Contribute to care coordination and service delivery within the Hospice at Home team.
Maintain accurate clinical records and uphold professional standards

Qualifications Essential
Registered Nurse RGN Level 1 Current NMC registration Evidence of continuing professional development CPD
Desirable
Specialist qualification or training in palliative or end of life care Mentorship/Practice Assessor qualification Training in communication skills e.g. difficult conversations, advance care planning
