Job summary
East Kennet Primary Care Network (PCN) is looking for a motivated Clinical Pharmacist Independent Prescriber to join our pharmacy team and support patient care at Ramsbury and Wanborough Surgery
This is a permanent role based at Ramsbury and Wanborough Surgery. The preferred working pattern is 22.5 hours per week, although we're open to discussing additional hours for the right candidate
You'll be part of a friendly and supportive PCN team, working alongside GPs, practice teams and other clinicians to support safe prescribing, medicines optimisation, structured medication reviews and care for patients with long term conditions
Salary will depend on experience, skills and prescribing qualification. We'll endeavour to offer a competitive package for the right candidate
Main duties of the job
The Clinical Pharmacist Independent Prescriber will support Ramsbury and Wanborough Surgery as part of East Kennet Primary Care Network, providing practical clinical pharmacy support to patients, GPs and the wider practice team
The role will focus on safe prescribing, medicines optimisation, structured medication reviews, long term condition support, medicines reconciliation and responding to medication queries
The postholder will work independently within their professional competence, with support from the wider multidisciplinary team. As an Independent Prescriber, you will prescribe within agreed scope of practice and local guidance
Main duties will include
Carrying out structured medication reviews
Supporting patients with long term conditions
Reviewing and reconciling medicines after hospital discharge
Responding to medication queries from patients and colleagues
Supporting safe and effective repeat prescribing
Providing advice on medicines and prescribing
Contributing to prescribing safety, audit and quality improvement work
Maintaining accurate clinical records

East Kennet PCN is a well established network made up of four GP practices, supporting a patient population of around 38,000 across a beautiful rural part of Wiltshire
Our member practices are Kennet and Avon Medical Partnership, Ramsbury and Wanborough Surgery, Old School Surgery Great Bedwyn and Burbage Surgery

Person Specification
Experience Essential
Experience with working as part of a team and actively supporting patients in various healthcare settings.

Experience of working within General Practice setting
Qualifications Essential
GPhC Registered Pharmacist Independent Prescriber
Desirable
Completion of relevant Centre for Pharmacy Postgraduate Education (CPPE) pathway
