Job summary
We are seeking an experienced Advanced Clinical Practitioner to join our newly formed Referral and Assessment Hub (RAH) in Manchester.
The RAH is an exciting and essential part of Manchester's journey to providing a fully transformed model of community care. The purpose of the Hub will be to provide swift triage and assessment of service users, adopting a system wide approach to identifying an onward pathway.
Main duties of the job
The ACP will work as part of an MDT to provide senior leadership and advice to practitioners and referrers within the Hub.
They will be experienced in holistic, trauma informed assessment and formulation, identifying goals and working in a solution focussed way.
The role will involve both direct and indirect working by seeing some service users in a clinic setting and providing advice and consultation to the wider team.
The role will be supported by a Consultant Psychiatrist, Pharmacist, Mental Health Practitioners, administrators and staff from the VCSFE sector.
As this is a new service, there is an expectation that this role will be key in developing the service to be safe, effective and innovative in the way it offers care.
