Job summary
The Band 4 Assistant Practitioner (AP) within the Adult Community Mental Health Team plays a key role in supporting the delivery of high-quality, recovery-focused care to adults experiencing mental health difficulties in community settings. Working under the supervision of registered practitioners, the Assistant Practitioner contributes to assessment, care planning, intervention, and review processes, ensuring service users receive timely, person-centred support aligned with NSFT values and clinical standards.
The post holder works as part of a multidisciplinary team (MDT) which may include nurses, social workers, psychologists, occupational therapists, psychiatrists, and peer support workers. They provide both clinical and practical support, helping individuals to manage their mental health, maintain independence, and improve overall wellbeing.
Main duties of the job Deliver defined aspects of care, including psychosocial interventions, monitoring mental state, and supporting care/recovery plans as both LCP and under supervision. Work collaboratively with service users, carers, and the wider MDT to support recovery and promote independence. Undertake routine assessments, risk observations, and contribute to care reviews, escalating concerns appropriately. Support service users with daily living skills, social inclusion, and engagement with community resources. Maintain accurate, timely clinical records in line with Trust policy and professional standards . Deliver health promotion and psychoeducation interventions where appropriate. Act as a key point of contact for service users, offering consistent support and guidance. Contribute to safeguarding processes and risk management plans.
