An exciting opportunity has arisen for a motivated and enthusiastic Trust Grade Doctor to join the Leeds Recovery Hubs Service.
Working within an innovative multidisciplinary model, you will provide high-quality, evidence-based medical care to adults with complex health needs in community intermediate care settings. The role supports older adults living with frailty, alongside younger adults with complex multimorbidity, long-term conditions and rehabilitation needs.
This is an excellent opportunity for a clinician with an interest in Community Medicine, Frailty, Rehabilitation, General Medicine or Integrated Care to develop their skills within a forward-thinking service focused on supporting recovery, promoting independence and preventing unnecessary hospital admissions.
The post offers a varied and rewarding clinical experience, working across organisational boundaries to deliver integrated, person-centred care that improves outcomes for patients and carers.
Main duties of the job
As a Trust Grade Doctor within the Leeds Recovery Hubs Service, you will:
Undertake comprehensive medical assessment and management of adults with complex physical health needs Provide clinical input for patients living with frailty, multimorbidity, chronic disease, functional impairment and rehabilitation needs Support rehabilitation and recovery through collaborative multidisciplinary care planning Contribute to admission avoidance pathways and facilitate timely, safe discharge planning Work closely with patients, carers and professionals to develop personalised management plans that maximise independence and quality of life Participate in multidisciplinary team meetings, case discussions and service development initiatives Provide clinical leadership and support to colleagues across community and intermediate care services Contribute to continuous service improvement and the ongoing development of the Leeds Recovery Hubs model

Title: Trust Grade Doctor - Community Older Peoples Medicine
Location: Leeds Recovery Hubs - 4 sites across Leeds
Hours of work: 15 hours per week (0.4wte) on a Wednesday and Friday
Fixed term contract for 6 months
Clinical Care
Undertaking comprehensive assessments of adults with complex medical, functional, psychological and social care needs
Assessing, diagnosing and managing a broad range of acute and chronic conditions commonly encountered within community and intermediate care settings
Supporting the management of patients with multimorbidity, frailty syndromes, falls, delirium, dementia, polypharmacy, chronic disease exacerbations, deconditioning and rehabilitation needs
Functional assessment and rehabilitation goal setting including developing person-centred management plans in collaboration with patients, carers and the multidisciplinary team
Supporting admission avoidance pathways and providing timely clinical review for patients at risk of hospital admission
Contributing to safe discharge planning, rehabilitation and continuity of care following hospital admission or episodes of acute illness
Participating in multidisciplinary team meetings, case reviews and care planning discussions
Maintaining accurate clinical documentation and ensuring effective communication with primary care, acute services, social care and community teams support available on-site / remote during service hours
Work under indirect supervision of senior clinicians (GP / Geriatrician)
Manage patients autonomously within competency, with clear escalation pathways
Responsible for recognising limits of competence and escalating appropriately
Recognition and management of acute frailty presentations including use of frailty assessment tools (e.g. Clinical Frailty Scale)
Palliative and advanced care planning discussions where appropriate
Polypharmacy review and deprescribing principles (with MDT/pharmacy support)
Escalation of deteriorating patients using NEWS2 / clinical judgement, including escalation to urgent care / ambulance / secondary care when required
Early recognition of safeguarding concerns in adults
Participation in incident reporting systems
Service Development
Participating in clinical audit, quality improvement and service evaluation projects.
Supporting the development and implementation of evidence-based clinical pathways and best practice guidance.
Contributing to the ongoing development of Leeds Recovery Hub services and integrated models of care for adults with complex healthcare needs.
Undertaking mandatory training and engaging in continuing professional development activities.
Relationships, system working and delivery
Building effective relationships with patients, families, carers and healthcare professionals across Leeds.
Working collaboratively with consultants, GPs, nurses, therapists, pharmacists, social workers, care coordinators and voluntary sector partners.
Attending and actively contributing to multidisciplinary team meetings and service meetings.
Communicating clinical information clearly, effectively and compassionately to patients, relatives and colleagues.
Use of shared electronic patient records, including SystmOne, PPM and Leeds Care Record

Person Specification
Qualifications Essential
MBBS, MBChB or equivalent medical qualification.
Desirable
Additional qualifications relevant to Medicine, Frailty, Rehabilitation or Community Care.
Clinical Knowledge and Skills Essential
Ability to assess and manage adults with a range of acute and chronic medical conditions. Ability to formulate management plans and make appropriate clinical decisions. Demonstrates awareness of own limitations and seeks senior support appropriately. Competent in the assessment of deteriorating patients and risk management. Ability to assess and manage patients in community or non-acute settings
Desirable
Experience of caring for patients with frailty, multimorbidity, rehabilitation needs or complex discharge planning. Knowledge of Comprehensive Geriatric Assessment (CGA), admission avoidance and integrated care pathways.
Experience Essential
Successful completion of Foundation Training (or equivalent postgraduate experience).
Desirable
Experience in Geriatric Medicine, General Internal Medicine, Acute Medicine, Community Medicine, Rehabilitation Medicine, General Practice, Care of the Elderly, Palliative Care or Emergency Medicine.
Communication Skills Essential
Excellent written and verbal communication skills. Ability to communicate effectively with patients, carers, relatives and healthcare professionals. Demonstrates empathy, compassion and patient-centred communication.
Desirable
Experience presenting clinical information in meetings, teaching sessions or case discussions.
Quality Improvement and Governance Essential
Understanding of clinical governance principles, patient safety and quality improvement. Commitment to audit, reflective practice and continuous improvement.
Desirable
Participation in audit, quality improvement projects, research, presentations or publications.
