Job summary
An Exciting opportunity to join our Integrated Heart Failure Service at NHS University Hospitals of Liverpool Group to support the heart failure service on the Royal Liverpool Site on a 37.5 hour basis over 7 days. We are seeking a highly motivated individual with excellent interpersonal and communication skills. The successful candidate will be responsible for supporting the band 7 clinical nurse specialists within the current team.
The candidate will be required to work closely with and communicate within the heart failure MDT. They will require consideration of skills to provide patient education, support to reduce symptoms and prevent unnecessary hospital admission/ re-admission and maximise the patients quality of life.
The role requires flexibility and awareness of heart failure, working within an evolving proactive , dynamic and committed multi disciplinary heart failure team.

Review and assessment of patients with heart failureo Administration of intravenous medicationo Care and maintenance of vascular access deviceso Peripheral cannulationo Checking daily weight and assessing if leg swelling has reduced and symptoms improvedo Signposting/referring to other community or acute serviceso Ordering/taking/reviewing blood samples/ investigationso Communication within the MDT and outside organisationso Manage and organise referralso Input to service database National heart failure audit

Clinical Practice Perform specialist assessment of patients care needs, plan, implement and evaluate caredelivery using clinical knowledge and clinical skills. To provide emotional and psychological support to patients and their carers. Perform point of care bloods, review and interpret results and escalate concerns. To be a specialist in the clinical procedure of peripheral venous cannulation. To identify opportunities and provide health education and promotion for all patients andtheir carers. To identify opportunities to promote informed choice and control for patients. Support patients and their families and ensure that they receive the required information to enablethem to self manage, lead or participate in their care delivery. To maintain accurate and legible patient records in accordance with Trust policy andnursing and midwifery council (NMC) guidelines. To be aware of the limitations of own knowledge, skills and competencies. Recognise changes in patients condition that require the intervention of others and refer onas appropriate. To support, assist and encourage the multi-disciplinary team in resolving patient problems,by the provision of new and innovative models of case management. To work within the NMCs Scope of Professional Conduct. Deliver training to patients/carers, medical and nursing staff in hospitalsettings regarding IV therapy and IV access.
