Bradford Teaching Hospitals NHS Foundation Trust is now recruiting a forward thinking, innovative and experienced specialist occupational therapist, to work in the Elderly Virtual Ward.
This post provides a fantastic opportunity for a driven, passionate individual to contribute to the development of new pathways in delirium and falls - alongside the day to day work of preventing hospital admission and providing right time, right place, assessments in the community. Main duties of the job
You will be a member of a dynamic multi-disciplinary team providing Urgent Community Response and Discharge to Assess interventions in people's homes. Working with a well-resourced, multi-disciplinary team you will be supported to work in a blurred boundary way, and develop new clinical skills.
It is essential that you are a car driver and have a Full UK Driving License, reasonable adjustments will be considered.

Experience Essential
Substantial post-graduate experience, including recent experience in the NHS Previous rotations within the Rehabilitation setting Experience with frail elderly with poor cognition/ dementia

Good communication and organisational skills Competent in core areas of Occupational therapy practice
