Job summary
An exciting band 6 opportunity for registered nurses who have experience in cardiology has arisen in our heart failure virtual ward which is part of Dudley Heart Failure Team. The successful applicant will be responsible forthe management of patients with heart failure over a7 dayservice. This may also include in-reaching into hospitalin order toprovide a seamless service.
This will involve direct face to face and virtual monitoring to provide acute patient management with a confirmed heart failure diagnosis in the community setting rather than hospital. The virtual ward will be a safe and efficient alternative to NHS bedded care that is enabled by technology. Patients will be admitted onto the virtual ward on acase by casebasis for up to a14 dayperiod.
The service aim is tofacilitatea reduction in patient length of stay; a reduction of hospital admissions/readmissions andassistearly discharge from hospital byexpeditingsafe andtimelypro-active support.
The post holder willbe responsible forthe delivery of comprehensive clinical assessment and care aimed at improving quality of life, supporting and empowering patients and their carers to manage their condition
Main duties of the job
The post holder will work as a specialist practitioner within the Dudley Heart Failure TeamVirtualWard,assistingin the provision of a comprehensive service toan allocatedcaseload of patients with Chronic Heart Failure. This may also include in-reaching into hospitaland out-reaching in the Communityin order toprovide a seamless service.
