Job summary

Care Coordinators play a vital role within the Primary Care Network (PCN) to proactively identify and work with people - particularly those who are frail, elderly, or living with long-term conditions - to provide effective coordination and navigation of care across health and care services.

Our Care Coordinators work closely with GPs, practice teams and wider multidisciplinary colleagues to support patients with complex needs. Acting as a central point of contact, Care Coordinators ensure that appropriate, timely support is available to individuals, helping them to understand and manage their conditions and ensuring their changing needs are recognised and addressed.

We are seeking a highly motivated individual who is passionate about supporting people, enjoys working collaboratively and wants to be part of a team to improve the way care is delivered to patients within our practices and linked care homes.

About us

Mid Norfolk Healthcare PCN is a collaborative network of five GP practices serving a predominantly rural population of around 50,000 patients across Dereham and the surrounding areas.

Our practices, Elmham Surgery, Mattishall & Lenwade Surgeries, Orchard Surgery, Shipdham Surgery and Theatre Royal Surgery, work closely together to deliver high-quality, accessible and patient-centred care for our local communities.

We have a strong focus on delivering personalised care, particularly for people with long-term conditions, frailty and complex health needs. Our dedicated Personalised Care Team brings together Care Coordinators, Health and Wellbeing Coaches and Social Prescribing Link Workers to support patients beyond traditional medical care.

The post holder will be based at Theatre Royal Surgery in Dereham, working as part of the practice team. However, you will benefit from being part of a supportive, experienced and forward-thinking team, sharing learning, best practice and peer support across the primary care network.

Job description Job responsibilities

The post holders, working closely with the PCN team, care homes, GP Practices and existing community services will:

Work collaboratively with other teams and services to maintain an effective and efficient service. Plan and organise work using own initiative, whilst being able to work as a valuable team member. Be able to make referrals to relevant service providers. Coordinate and where required chair MDT meetings, having proactively prepared any actions prior to the meeting and ensure all relevant clinical and non-clinical staff are present. To promote and facilitate integration within the GP and community teams. To provide and receive sensitive information about difficult or complex matters always respecting confidentiality. Create detailed and clear minutes from the meetings and forward to all relevant parties with actions highlighted to individuals. Where required, be able to offer appropriate support and guidance to patients and families / carers. Receive, record, and collate information and maintain accurate electronic records of patient care and planned service provision, including hospital admission and discharge information. Signpost service users and carers to relevant services. Contribute to quality audits. Cooperate and support colleagues to share best practice, complete tasks to maximise effectiveness and team performance. Take part in appropriate training and take responsibility for own personal development. Contribute to the development of the role and systems, to effectively enhance the service provided and improve outcomes.To assist patients to be better prepared to have conversations on shared decision making and to improve awareness of shared decision making and related support tools. To provide patients with high quality, easy to understand information to assist them in making choices about their care. To support in the delivery of enhanced services and other service requirements on behalf of the PCN Support delivery of QOF or cost effectiveness initiatives Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner. - A-level NVQ 3 or equivalent experience in admin/customer service environment

Experience Essential

Excellent IT skills, including strong working knowledge of Microsoft Office packages (e.g. Word, Excel, Outlook) and clinical systems , such as SystmOne or EMIS. Excellent typing skills. Experience of working with sensitive and confidential information, with a clear understanding of data protection and information governance requirements. Proven administrative and organisational experience, with the ability to manage competing priorities and maintain accurate records. Strong communication and interpersonal skills, with the ability to build effective working relationships with patients, colleagues and partner organisations. Experience of working within health, social care, or a community-based setting, supporting individuals with a range of needs. Understanding of Clinical Governance, including safeguarding, confidentiality and safe working practices. Ability to work in a person centred and strengths-based way, supporting individuals to be actively involved in their care. Experience of working within a team and/or across organisational boundaries, supporting collaborative or multidisciplinary working. Ability to use initiative and work independently, with a proactive and flexible approach.

Desirable

Experience of working in primary care/GP Practice. Working knowledge of the management of databases and/or analysis of health related data. Experience of working within Multidisciplinary teams. Knowledge and understanding of relevant health and social care legislation and initiatives.
