Job summary
An exciting opportunity has arisen for an enthusiastic and motivated Registered Mental Health Nurse to join the Brent Older Adults Community Mental Health Team on a part-time basis.
We are a friendly and supportive multidisciplinary team providing specialist mental health care to older adults in the diverse London Borough of Brent. We support people with a range of mental health needs, including functional mental illness, dementia, and complex presentations, promoting independence within the community.
This role offers the opportunity to develop specialist community mental health skills while working autonomously and as part of an experienced MDT. You will work collaboratively with service users, carers, families, primary care, social care and voluntary sector partners to deliver high-quality, person-centred care.
As a Community Mental Health Nurse, you will undertake comprehensive mental health assessments, manage a defined caseload, coordinate care, contribute to risk management and safeguarding, and deliver evidence-based interventions to promote recovery, wellbeing and independence. You will build therapeutic relationships with service users and carers, supporting them through complex and challenging circumstances.
Main duties of the job
Take responsibility for a complex caseload of older adults with severe and enduring mental health conditions, including dementia, functional mental illness, comorbid physical health needs and complex psychosocial difficulties.
Act as Care Coordinator within the Care Programme Approach (CPA) / Dialog+ framework, ensuring holistic assessment, collaborative care planning, and review of recovery-focused care plans.
Work collaboratively with MDT colleagues and external agencies including psychiatry, psychology, social care, primary care, memory services, inpatient teams and voluntary sector partners.
Participate in duty systems including triage, urgent assessments and crisis response, ensuring timely intervention.
Contribute to safeguarding processes in line with Trust and local authority policies.
Provide supervision, mentorship and support to junior staff, students and support workers, promoting learning and best practice.
Participate in MDT meetings, case discussions and service development, contributing to continuous quality improvement.
Maintain accurate and timely clinical documentation in line with Trust and NMC standards.
Promote least restrictive practice and support independence within the community wherever possible.
Engage in ongoing professional development, supervision and NMC revalidation.

The post holder will be responsible for delivering high-quality, person-centred community mental health care within the Older Adults CMHT. This includes undertaking comprehensive biopsychosocial assessments, including mental state and risk assessments, and contributing to the development, implementation and review of individualised, recovery-focused care plans.
The role involves managing a defined caseload of older adults with severe and enduring mental health conditions, including dementia and functional mental illness, working as Care Coordinator within relevant care coordination frameworks. The post holder will work autonomously while remaining accountable within a multidisciplinary team setting.
Key responsibilities include coordinating care in collaboration with service users, carers, families and partner agencies such as primary care, social services, psychology, psychiatry and voluntary sector organisations. The post holder will deliver and support evidence-based interventions, promote recovery, independence and wellbeing, and ensure timely and effective risk management and safeguarding processes.
The role includes participation in duty systems such as triage, urgent assessments and crisis response, ensuring safe and appropriate interventions. The post holder will contribute to MDT discussions, clinical decision-making, and service development activities, while maintaining accurate and timely clinical records in line with Trust and NMC standards.
