Main duties of the job
The Practice Nurse will work as part of the multidisciplinary team at Montpelier Surgery to provide high-quality, evidence-based nursing care to our patient population. The post holder will deliver a range of treatment room and chronic disease management services, health promotion activities, screening programmes, and preventative healthcare interventions, contributing to the delivery of safe, effective, and patient-centered care.
The Practice Nurse will work collaboratively with GPs, the Healthcare Assistant, Clinical Pharmacist, and wider Primary Care Network colleagues to support patients in managing their health and wellbeing.

Our multidisciplinary team consists of 7 GP partners and salaried doctors, 1 Practice Nurse, 1 Healthcare Assistant, and a Senior Clinical Pharmacist, supported by additional ARRS-funded roles through our Primary Care Network.

Key Responsibilities
Clinical Practice
Deliver clinical nursing services including: Cervical cytology screening Wound care and dressings Suture and clip removal ECG recording Venepuncture Blood pressure monitoring Injections and immunisations Provide travel health assessments, advice, and vaccinations in accordance with current national guidance and practice protocols. Deliver routine and seasonal vaccination programmes, including influenza, pneumococcal, shingles, RSV, and COVID-19 vaccinations where applicable. Undertake NHS Health Checks and health promotion activities. Support the management of long-term conditions including: Diabetes Hypertension Asthma COPD Cardiovascular disease Assist with annual reviews and recall programmes. Provide lifestyle advice and support patients with smoking cessation, weight management, physical activity, and healthy living initiatives. Recognise and respond appropriately to safeguarding concerns. Maintain accurate and contemporaneous patient records using the clinical system. Ensure infection prevention and control procedures are followed at all times.
Patient Care
Deliver care that is patient-centred, compassionate, and evidence based. Promote patient independence and self-management of long-term conditions. Communicate effectively with patients, relatives, carers, and colleagues. Ensure patients are treated with dignity, respect, and confidentiality.
Team Working
Work collaboratively within the multidisciplinary team. Liaise with external healthcare professionals and community services where appropriate. Participate in clinical meetings and practice development activities. Support colleagues and contribute to maintaining a positive and supportive working environment.
Quality and Governance
Work within NMC standards, professional codes of conduct, and practice policies. Participate in clinical audit, quality improvement initiatives, and service development. Maintain professional registration and fulfil continuing professional development requirements. Report incidents, significant events, and safeguarding concerns in line with practice procedures.
Administrative Responsibilities
Maintain accurate clinical records and coding. Contribute to recall and screening programmes. Manage clinical stock levels and ensure equipment is maintained and used safely. Support achievement of Quality and Outcomes Framework (QOF) and other contractual requirements. Professional Responsibilities
Maintain registration with the Nursing and Midwifery Council (NMC). Work within the scope of professional competence. Participate in mandatory training and continuing professional development. Adhere to all practice policies, including information governance, confidentiality, safeguarding, equality and diversity, and health and safety.
Person Specification
Qualifications Essential
Registered Nurse with current NMC registration. Experience working in primary care or a community healthcare setting. Competence in cervical cytology. Competence in immunisations and vaccinations. Experience of chronic disease management. Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. IT literacy and experience using electronic patient records.
Desirable
Qualification in Chronic Disease Management. Independent Nurse Prescriber qualification. Experience of asthma, COPD, diabetes, or cardiovascular disease reviews. Experience of clinical audit and quality improvement projects. Knowledge of SystmOne.
