Main duties of the job
An exciting opportunity is available for a dedicated nurse to deliver high-quality clinical care to patients in their own homes. Working under the direction of the Community Nurse Team Leader, you will be responsible for assessing and managing the health needs of a defined patient population, including those with long-term conditions, complex nursing needs and end-of-life care, supporting patients to remain in their preferred place of care wherever possible.
You will make complex clinical decisions to manage patient care safely and effectively, particularly within an out-of-hours environment. This includes understanding escalation processes, triaging all communications using electronic patient records, and coordinating workload across teams to ensure care is prioritised according to clinical need. Effective delegation and demand management are essential, alongside the ability to support long day staff and allocate work across sectors.
The role also involves providing reassurance, advice and practical support to patients, families and carers, ensuring high standards of compassionate care. You will contribute to maintaining staff safety by supporting lone working systems and processes. This is a varied and rewarding role requiring strong clinical judgement, organisational skills and the ability to work autonomously while remaining part of a wider, supportive team.
