Job summary
We are looking for an experienced Practice Nurse to join our friendly established team at a lovely practice in Plaistow E13. Must have experience of working in General Practice and have UK registered nurse with significant post registration experience
Working directly with the nursing team and strictly in accordance with specific practice guidelines and protocols, the nurse will assist the practice clinical team in the provision and delivery of prescribed programmes of patient care.
Main duties of the job
Qualified Cervical Smear sample taker
Phlebotomy
Qualified in Childhood/ Routine vaccines, contraception, Chronic Disease management and would care management
Managing long term conditions
Processing and management of laboratory samples requested by GPs
Vaccine and cold chain storage, monitoring and recording
Preparing and maintaining environments and equipment before, during and after patient care interventions.
Assisting in the assessment and surveillance of patients health and well-being
Good knowledge on quality outcomes framework
Helping to raise awareness of health and well-being and how it can be promoted

Essex Lodge Surgery is an expanding PMS and teaching practice serving approximately 17,000+ patients in London Borough of Newham with 2 GP Partners, 4 Salaried GPs, 2 regular locum GPs, 1 GP Registrar, 3 Practice Nurses, , 3 Clinical Pharmacists, 1 Prescribing Pharmacist, 1 physiotherapist, 1 Paramedic, 1 Menatl Health Nurse Practioner and a friendly admin and managerial team. Practice is located in Plaistow, London (E13 0AS) less than 10 minutes walk from Plaistow underground station.

We are a busy, 17,000+ general practice in beautiful East London looking for a experienced Practice Nurse with a flexible approach, can-do attitude and a strong commitment to the aims and values of primary care to join our friendly, professional team. The practice has a good CQC rating in all areas and prides itself on maintaining high standards of care.
Key responsibilities include:
Management and review of patients with long-term conditions, including diabetes, COPD, asthma, hypertension, mental health and other chronic diseases.Conducting annual reviews and care planning in line with local and national guidance.Performing health assessments, screening, and preventative healthcare interventions.Providing patient education and support to improve health outcomes.Maintaining accurate clinical records and contributing to quality improvement initiatives.Working collaboratively with GPs, pharmacists, healthcare assistants, and the wider multidisciplinary team.Essential Requirements:
Registered Nurse (NMC registered).Experience working in General Practice or Primary Care.Knowledge and experience of long-term condition management, particularly diabetes and COPD.Excellent communication and organisational skills.Competent in clinical assessment and patient education.Experience with QOF, care planning, and chronic disease management templates is desirable.
Finally the ability to build a professional rapport with patients, to demonstrate confidence in your ability and ensure any investigations are comfortable and calm.
