Job summary

We are seeking an experienced and motivated Diabetes Specialist Nurse to join our primary care team. The successful candidate will provide expert clinical management for patients with diabetes, supporting early intervention, effective disease management, and the prevention of long-term complications.

Main duties of the job

Working collaboratively with GPs, practice nurses, healthcare assistants, and the wider multidisciplinary team, the post holder will deliver specialist diabetes clinics, provide patient education, support self-management, and contribute to the ongoing development of diabetes services within the practice.

A key part of this role will also include undertaking general practice nursing duties, working flexibly within the wider nursing team to support core general practice services, including chronic disease management, long-term condition reviews, vaccinations, and routine treatment room activities.

We are a busy and well-established teaching GP practice located in Luton town centre, serving a diverse patient population of approximately 17,000 patients. The practice is led by two GP partners and supported by a strong multidisciplinary team.

The post holder will work as a Diabetes Specialist Nurse within our busy GP practice in Luton, serving a population of approximately 17,000 patients.

The role involves delivering high-quality diabetes care, including patient assessment, initiation and adjustment of treatment, and ongoing monitoring in line with current guidelines. A key focus will be supporting patients with self-management and reducing diabetes-related complications through education and regular clinical review, including structured diabetes reviews and annual assessments.

The post holder will work closely with the wider clinical team, including GPs, physician associates, clinical pharmacists, paramedics, nurses, healthcare assistants, and administrative staff, to support coordinated patient care.

The role includes participation in QOF-related work, including diabetes reviews, recall systems, and supporting achievement of practice quality targets.

In addition, the post holder will support general practice nursing duties, including chronic disease management, long-term condition reviews, immunisations, and treatment room work.

Person Specification

Experience Essential

Experience managing patients with Type 1 and Type 2 diabetes in a primary care, community, or specialist setting. Experience initiating, monitoring, and adjusting diabetes treatments, including insulin therapy. Experience undertaking comprehensive clinical assessments and developing personalised care plans. Experience maintaining accurate clinical records and using electronic patient record systems.

Desirable

Experience working within general practice or a primary care setting. Experience supporting achievement of Quality and Outcomes Framework (QOF) targets and long-term condition reviews. Experience managing patients using diabetes technologies, including continuous glucose monitoring (CGM) systems and insulin pumps. Experience supporting patients with complex diabetes and multiple long-term conditions. Knowledge of Systmone

Qualifications Essential

Registered Nurse with current registration with the Nursing and Midwifery Council (NMC). Evidence of continuing professional development relevant to diabetes care. Diabetes qualification at diploma level or equivalent specialist diabetes training and experience.

Desirable

Independent/Supplementary Nurse Prescriber qualification. Degree-level qualification or postgraduate study in diabetes care or a related field. Membership of a recognised professional diabetes organisation, such as Diabetes UK or the Primary Care Diabetes Society.
