The role of an Assistant Practitioner in the haemodialysis Department entails a higher level of competency. The post holder will be responsible for performing a range of identified tasks specific to the home haemodialysis service, while playing an active role in the multidisciplinary team.
The role is designed to support the delivery of high-quality, research-informed care to patients involved in Home Haemodialysis, Shared and Self-care, while also offering this role holder the opportunities for professional development and growth.

Main duties of the job
The Assistant Practitioner will support the Registered Nurse in the planning, implementation, and evaluation of nursing care within the Home Haemodialysis, Shared Care, and Self-Care areas. The role involves making a direct contribution to the delivery of holistic patient care.
Actively participate in communication within the multidisciplinary team (MDT), collaborating with other healthcare professionals to ensure holistic care and maintain patients' wellbeing. Duties will include ordering dialysis supplies, liaising with local councils regarding approval for dialysis installations, and referring patients to relevant services according to their individual needs, including dietitians, pharmacists, and the vascular access team.
Responsible for supporting aspects of patient training and will take an active role in carrying out clinical tasks delegated by the Registered Practitioner.
Contribute to the patients' training to an independent level.

Skills Essential
Ability to communicate effectively in a variety of settings with patients who have differing levels of understanding of health, illness, and self-care. Demonstrate a flexible approach to work Demonstrates patience and a holistic approach when training patients, ensuring training is delivered at an appropriate pace and tailored to individual needs.
Desirable
Ability to organise workload effectively and prioritise patient care needs according to clinical urgency, service demands, and patient safety Ability to work on own initiative with minimal direction from registered practitioner. Basic IT skills and willingness to develop those skills. Understanding of fluid balance, weight monitoring, and dialysis parameters
Qualifications/Training Essential
Foundation Degree in Health and Social Care Practice Renal Unit training programme based on Renal Band 4 competencies
Desirable
Level 3 apprenticeship and QCF as identified by department (or equivalent qualification)
Previous Experience Essential
Extended experience within renal and general healthcare settings, demonstrating understanding of renal disease and Haemodialysis care
Knowledge Essential
Demonstrates a good knowledge of dialysis and a clear understanding of the dialysis process, including principles of haemodialysis, vascular access types, and safe delivery of treatment. Awareness of vascular access care (AV fistula, grafts, central venous catheters)
Desirable
Ability to recognise and report patient deterioration promptly
Other Requirements Essential
Demonstrates understanding of equality, diversity, and inclusion (EDI) principles and their application in healthcare practice, policy, and service development.
