Memory Assessment Service
Non Medical / Independent Prescriber, Nurse Advanced Practitioner

Our Memory Assessment and Therapy team (MATS) are seeking a Band 7 Prescriber to join their collaborative team.
MATS is the front door to all of our West Suffolk referrals for memory assessment, affecting the national dementia diagnostic rate data. The team provides a community-based service to adults with a memory issue. We are soon to be re-named as a Dementia Assessment Service to align with national expectations. We screen and triage all referrals into the service, and signpost to appropriate services if required. Referred people can expect to receive a service from MATS within 12 weeks as per our KPI's. We have been transparent about delays to assessment due to having a high number of referrals, and the team are working towards a positive trajectory within the coming months.
The MATS team is based in Bury St Edmunds but works across the west of the county. You will be supported by our MDT and the wider older people's network.

Main duties of the job
Complete holistic mental health assessments with Medication review and prescribing.
Undertake the assessment, planning and delivery of dementia diagnosis and prescribing, if appropriate, with minimal supervision, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment. Contribute to the wider functioning of the memory assessment services and take a lead role in shaping positive changes within the team. Be responsible for accurate recording of all patients outcomes within the SystmOne EPR. Assist with the wider agenda within the transformation work of the Trust
The successful candidate will join a service with a positive team culture, focused on learning, developing, improving, and supporting each other as well as our service users.
General responsibilities include:
- Having a good understanding of the national dementia agenda
- Development of robust relationships with partner agencies and services.
- You will be expected to be able to work autonomously and manage your time well.

Person Specification
Qualifications Essential
Non-Medical Independent Prescriber RMN or equivalent professional qualification
Experience Essential
Extensive experience at which has required supervision and mentorship of others Extensive experience of practice within clinical area with proven track record in delivering high quality care.
Desirable
Teaching or training experience or qualification.
Skills Essential
Confident manner. Approachable, tactful and diplomatic. Ability to problem solve.
