Job summary
Whitewater Health is seeking an experienced and highly motivated Advanced Clinical Practitioner (ACP) to join our dynamic and supportive team. This is an opportunity to work in a forward-thinking and patient-centred GP practice, committed to delivering high-quality care within the local community. We are rapidly evolving with implementation of new technology into clinical practice to enhance patient care and reduce admin burden. You will be joining our team of supportive Advanced Clinical Practitioners who work alongside our full GP team.
We are seeking an additional ACP to join our team on a 28 hour contract, you must be able to work a Monday and Tuesday.
Main duties of the job
The successful candidate will be a qualified Advanced Clinical Practitionerwith aMSc in Advanced Clinical Practice. You will work autonomously, providing expert clinical assessment, diagnosis, treatment, and management of patients presenting with undifferentiated conditions in a primary care setting.
You will be expected to manage acute, urgent, and chronic presentations from both adults and children, demonstrating advanced clinical decision-making, critical thinking, and clinical reasoning skills.
Key Responsibilities
Undertake comprehensive clinical assessments of patients presenting with a wide range of undifferentiated conditions.
Diagnose and initiate appropriate care, including prescribing and referrals, in line with current guidelines and best practice.
Work collaboratively within a multidisciplinary team to ensure holistic and seamless care for patients.
Contribute to the ongoing development of clinical services and protocols within the practice.
Provide clinical leadership and mentorship to colleagues where appropriate.
Essential Criteria
MSc in Advanced Clinical Practice
Independent prescriber (V300).
Significant experience working in Primary Care.
Proven ability to manage undifferentiated presentations in both adult and paediatric populations.
Excellent communication, organisational, and interpersonal skills.

The post-holder is an Advanced Clinical Practitioner (ACP).
They have been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.
The ACP provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment.
The post-holder works at the level of advanced clinical practice and has developed and can evidence the underpinning competencies applicable to the specialty or subject area, i.e., the knowledge, skills and behaviours appropriate to the healthcare professionals setting and job role.
ACPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. An ACP is autonomous in making decisions based on assessment, diagnosis and the interpretation of test results and will initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.
