Job summary
We are seeking an enthusiastic and motivated Practice Nurse to join our friendly and supportive multidisciplinary team. This is an excellent opportunity for a registered nurse who is passionate about delivering high-quality, patient-centred care within a busy GP practice.
The successful candidate will work closely with GPs, healthcare assistants, pharmacists, and administrative colleagues to provide a comprehensive range of nursing services to our patient population. We are looking for an individual who is committed to promoting health and wellbeing, supporting patients with long-term conditions, and contributing to the ongoing development of primary care services.
Main duties of the job
The post holder will be responsible for delivering a range of nursing services, including:
Management and review of patients with long-term conditions such as diabetes, asthma, COPD, hypertension, and cardiovascular disease.Cervical cytology screening.Childhood and adult immunisations and vaccinations.Health promotion, screening, and lifestyle advice.NHS Health Checks.Venepuncture and other clinical procedures.Infection prevention and control activities.Maintaining accurate clinical records and documentation.Working collaboratively within the multidisciplinary team to provide safe, effective, and evidence-based care.Supporting quality improvement initiatives and achievement of practice objectives.

To provide and maintain a high standard of nursing care for patients attending the practice, working within professional guidelines and practice policies to deliver safe and effective clinical services.

Person Specification
Experience Essential
Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Knowledge of infection prevention and control principles. Understanding of safeguarding responsibilities. Good organisational and time management skills. Competent IT skills. Willingness to learn and adapt in a fast paced healthcare environment.
Desirable
Knowledge of Quality and Outcomes Framework (QOF). Understanding of current NHS and Primary Care priorities. Experience of working in a GP practice
Qualifications Essential
Registered Nurse qualification and registration with The Nursing and Midwifery Council with a valid NMC PIN Experience delivering vaccinations and immunisations. Experience undertaking clinical assessments and delivering patient-centred care. Experience maintaining accurate clinical records. Ability to work effectively in a demanding environment. Compassionate and patient-centred approach to care
Desirable
Previous experience working in General Practice or Primary Care. Experience managing long-term conditions. Experience using EMIS Web clinical system.
