Main duties of the job
The Role
As an integral member of our clinical team, you will work alongside GPs, Practice Nurses and other healthcare professionals to provide safe, effective and person-centred care. Your responsibilities will include:
Undertaking clinical observations and monitoring patients' health needs Performing routine clinical procedures where appropriately trained, such as: wound care, ECGs, phlebotomy, vaccinations and drug injections, ear irrigation, reviews: annual diabetes, dementia, heart failure, COPD, asthma, cytology, and health checks. Supporting the management of patients with long-term conditions Providing health promotion and lifestyle advice to patients Assisting with recall programmes and preventative healthcare initiatives Maintaining accurate clinical records and documentation Working within your scope of practice and escalating concerns appropriately Contributing to quality improvement activities and service developmentIf you're looking to develop your career within a practice that values teamwork, innovation and outstanding patient care, we'd love to hear from you.

Qualifications Essential
Nursing degree Membership of a professional body NMC (Nursing Midwifery Council)
Specific skills and competencies Essential
Registered Nurse associate with NMC pin number Cytology trained. Childhood immunisation trained. Demonstrates skills in literacy, written, verbal and an ability to effectively use electronic information and computer technology. Demonstrates an understanding of the importance of quality of care. Demonstrates an understanding of the role of a Nursing Associate and Healthcare Assistant. Able to work under the direction of a registered practitioner. Ability to work efficiently, effectively and professionally in a multi- disciplinary team. Ability to work under own initiative. Flexible with the ability to work within a multi-disciplinary team and across multiple sites. Experience with administering injections
Experience Essential
Clinical To support the practice nursing team in the delivery of nursing services, including: undertaking patient health checks, e.g. ECGs, BPs, BMI checks etc, carrying out baseline observations such as pulse oximetry, blood pressure, temperature, pulse rate, undertaking health reviews and screenings, e.g. COPD, asthma, diabetes, and other chronic diseases, cervical screenings, administering vaccinations and phlebotomy, wound care, ensure specimens are recorded and ready for onward transportation, other associated tasks within the capabilities of the postholder. Provide clinical care (including associated administration) via face to face, telephone and digital consultations. Collaborate as needed with internal stakeholders, to facilitate coordinated care for patients. Refer patients for further evaluation or treatment as needed. Chaperone as needed for patients. Education and Health Initiatives Support the practice nurse with health promotion programmes. Participate in health promotion events, screenings, immunisation campaigns and community outreach programs aimed at improving the health and well-being of individuals and families in the community. Provide health education to patients and their families on topics such as preventive care, chronic disease management, medication adherence, and lifestyle modifications. Provide medication education, and monitoring for potential drug interactions or adverse effects. Safeguarding Be familiar with safeguarding procedures for the practice. Escalate any safeguarding concerns without delay to the practices Designated Safeguarding Lead and/or Practice Manager/Lead. Clinical Governance, Health and Safety, and Complaints Management Working closely with the Nurse Lead to fulfil audits and any nursing administration. To have a good knowledge of infection control, to reduce risk of health care acquired infections. To deliver the role in compliance with regulatory standards. To adhere to all Company Policies and Procedures. To ensure data protection and confidentiality are maintained at all times. To take reasonable care for the health and safety of themselves and others - ensure work areas are tidy and free from hazards. To liaise with the relevant Practice Manager/Lead regarding any incidents or complaints and follow any action as directed. Personal Development Ensure that personal, mandatory, or organisational training is completed and up to date. Maintain own skills, knowledge, and competence for the role. Participate in annual appraisals and any subsequent learning/change in practice.
