Job summary
We are looking to recruit an enthusiastic Occupational Therapist to join our expanding multidisciplinary Dementia service within Hounslow. The candidate must have good knowledge and experience of working with people living with Dementia along with the skills to work as part of a multidisciplinary community team. You will work with people living in their own homes to live well with Dementia, reduce the risk of crisis, hospital admission and the need for increased formal care.
Main duties of the job
You will work with patients on the caseload to remain independent within their own home environment, providing proactively support, clinical interventions/support and care planning.
The service works to reduce unplanned hospital admissions, prevent crisis, maintain wellbeing and enable people to maintain their own wellbeing whilst living with Dementia.
Successful candidates for the post will need to be self-motivated, forward thinking and committed to learning. You will proactively build professional relationships and work in collaboration with the multidisciplinary team.
Complete comprehensive holistic assessment using clinical skills and knowledge, to interpret a range of complex clinical findings to develop appropriate, treatment / care plans intervention and evaluation of the patients in the service.
Use clinical knowledge to identify changing or deteriorating health conditions or function and to educate patients and families to recognise these signs and refer to appropriate health services.
Identify and address any equipment needs that will promote functional independence and safety within the home environment. This includes to assist carers and family members.

Qualifications Essential
Evidence of postgraduate personal/ professional development Current registration with the HCPC Degree or equivalent in Occupational Therapy
Experience Essential
Relevant post graduate practical experience as a band 5/6 Occupational Therapist, including experience working with older people / people with dementia Broad range of core clinical specialities including Orthopaedics, Medical, Neurology, Mental Health and Older People. Demonstrable post-graduate experience in a relevant area working with people with long term conditions (dementia) Experience of working as part of an Occupational Therapy team and/or multi-disciplinary team.
Desirable
Experience of the workings of a community occupational therapy service.
Skills & Abilities Essential
