Job summary
Battersea PCN Ltd is looking for a motivated, enthusiastic paramedic with an interest in primary care working to join our existing paramedic team. This is an opportunity to be involved in developing new ways of working in Primary Care, with our friendly, forward thinking practice teams. Whilst previous experience in a GP setting is an advantage we are happy to offer training and support to the right candidate.

The Paramedic Practitioner will work autonomously within the GP practices, using their enhanced clinical assessment and treatment skills to provide first-point-of-contact for patients presenting with undifferentiated, undiagnosed problems such as minor illness or injury, and assessment of patients with long-term conditions. We provide a supportive clinical and educational environment, working collaboratively with GPs and other clinical colleagues.
The successful candidate will have excellent communication skills and will work closely with our GPs, Clinical Pharmacists, Nurses, and administrative and reception teams. You will have good time-management skills and be able to prioritise tasks and work independently.

Main duties of the job
Assess and triage patients and provide appropriate treatment or make necessary referrals to other members of the primary care team. Assess patients presenting with acute illness or for review of chronic disease conditions Undertake home visits for acute illness or chronic disease management Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly including those with complex needs Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan Advise patients on general healthcare an promote self-management where appropriate, including signposting patients to other community or voluntary services Support patients to adopt health promotion strategies that promote health lifestyles, and apply principles of self-care Perform specialist health checks and reviews; with investigatory procedures as required. Undertake the collection of pathological specimens including intravenous blood samples, swabs etc and interpret results Communicate at all levels across internal and external organisations ensuring that an effective, patient-centred service is delivered. Independent prescribing. We will consider applications from Paramedic Practitioners who have not yet achieved this they would be supported to do so in time.

Battersea PCN Ltd comprises of 5 GP practices in a compact are of North Battersea - Bridge Lane Group Practice, Battersea Fields Practice, Queenstown Road Medical Practice, Lavender Hill Group Practice, Battersea Rise Group Practice - with a combined population of approximately 58,000 patients. Assess and triage patients, including same day triage, and as appropriate provide treatment including prescribing medication following policy, local and national guidelines and local care pathways, or make necessary referrals to other members of the primary care team. Assess patients presenting with acute illness or for review of chronic disease conditions Undertake home visits for acute illness or chronic disease management Diagnose, plan implement and evaluate treatment and interventions and care for patients presenting with undifferentiated diagnoses. Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly Assess, diagnose, plan, implement and evaluate interventions and treatments for patients with complex needs including requesting and interpreting appropriate test results Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan Advise patients on general healthcare an promote self-management where appropriate, including signposting patients to other community or voluntary services. Work with patients in order to support compliance and adherence to prescribed treatments Provide information and advice on prescribed and over-the-counter treatments Support patients to adopt health promotion strategies that promote health lifestyles, and apply principles of self-care Perform specialist health checks and reviews and perform investigatory procedures as required. Undertake the collection of pathological specimens including intravenous blood samples, swabs etc Support the delivery of anticipatory care plans and lead certain community services eg, monitoring blood pressure and diabetes risk of elderly patients living in supported housing Provide and alternative model to urgent and same-day GP home visits Communicate at all levels across organisations ensuring that an effective, patient-centred service is delivered. Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required. Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice. Experience Essential
5 year post NQP qualification experience working as a paramedic within the NHS
Desirable
Experience working in a GP practice as a paramedic, working autonomously to provide first-point-of-contact for patients presenting with undifferentiated, undiagnosed problems such as minor illness or injury, and assessment of patients with long-term conditions. Demonstrable experience manage a longitudinal caseload (including follow-up and outcome evaluation as well as single episode care) Experience in service development, pathway design and leadership at practice or PCN level Previous supervisory or mentorship experience Experience delivering education or training sessions to colleagues or wider MDT groups

Qualifications Essential
HCPC registered paramedic with no current issues under investigation Educated to degree/diploma level in paramedicine or equivalent Level 3 Safeguarding (children) Evidence of progress through, or completion of the First Contact Practitioner Roadmap to Practice (stage 1&2)

Independent Prescriber qualification (or working towards) Post-graduate/Masters level qualification or equivalent advanced practice training
