Job summary
We are looking for a motivated and committed team player to join our very friendly and supportive team in Sevenoaks Kent.
We required 6 sessions on a Monday, Thursday and Friday. Our multidisciplinary team including GP's, ACP, Nurse, HCA, Pharmacist and knowledgeable admin team work collaboratively to deliver high-quality, innovative and compassionate care.
We are always happy to support GPs in their special interest.
Main duties of the job
Provide high-quality, patient-centred clinical care to our diverse population, including acute, chronic disease, and preventive care.
Undertake face-to-face, telephone, online consultations and rare home visits as we have a low home visit rate.
Work collaboratively with the multidisciplinary team to ensure effective, holistic care.
Participate in practice meetings, clinical audits, quality improvement initiatives, and significant event reviews
Manage administrative tasks related to patient care, including referrals, pathology results, and correspondence.

Job description Job responsibilities
Deliver face-to-face, telephone, and video consultations to manage acute and chronic health conditions for a diverse patient caseload
Make autonomous clinical decisions, including physical examinations, arranging investigations (e.g., blood tests, scans), and referring to secondary care or specialists
Issue computer-generated acute and repeat prescriptions in line with national and local practice formularies
Conduct visits to patients' homes or care homes as part of the daily rota
Identify disease risk factors and early signs of illness through screening and health education
Process lab results, hospital correspondence, Docman
Participate in clinical audits, Quality and Outcomes Framework (QOF) targets, and significant event reviews to improve patient care standards, clinical governance, and continuous improvement initiatives
Attend practice and clinical meetings and work within a multidisciplinary team including nurses, pharmacists and Advanced Care Practitioners
