We are a friendly and forward-thinking GP practice in South Sefton looking to recruit a motivated and patient-focused Practice Nurse to join our supportive multidisciplinary team.

Main Duties
The role will include a mix of
Chronic disease management e.g. asthma, COPD, diabetes
Cervical cytology (smears)
Immunisations and vaccinations
Health promotion and lifestyle advice
Supporting QOF and enhanced service delivery
About You
We are looking for someone who:
Is a registered nurse NMC
Has experience in primary care desirable but not essential
Is confident running clinics and working independently
Has excellent communication and patient care skills
Is committed to delivering high-quality, compassionate care

Qualifications Essential
Role purpose: Provide high-quality nursing care in a GP practice, supporting patient health, prevention, and chronic disease management. Key duties: Deliver routine nursing clinics (e.g. immunisations, wound care, cervical screening). Manage long-term conditions (e.g. diabetes, asthma, hypertension). Perform health checks and screening. Provide health education and lifestyle advice. Maintain accurate patient records. Support infection prevention and clinical governance. Requirements: Registered Nurse (NMC). Primary care or community experience desirable. Strong communication and organisational skills. Reports to: GP/Practice Manager.
