Are you an experienced Occupational Therapist looking for a Community role? We have a new and exciting opportunity for a band 6 Occupational Therapist to join the team based in Wokingham.
You will be responsible for the provision of specialist Occupational Therapy care for patients in the community under the Community Based Neuro Rehab Team and Earlier Support Discharge Team for Stroke.Youll work closely with the Multidisciplinary Team (MDT); this is to ensure the service is run in an efficient and effective manner and you will also provide Occupational Therapy within other community settings as required.
Main duties of the job
Responsible for the provision of specialist Occupational Therapy care for patients under the care of the Community Based Neuro Rehab Team.To ensure patients under your management receive a high standard of clinical care and support.
Work closely with the Occupational Therapy in-patient Senior Therapists & other neurological specialists and the wider team to ensure Occupational Therapy services provided are of a high standard, responsive to patient and multidisciplinary team (MDT) needs and to ensure the service is run in an efficient and effective manner.
Undertake comprehensive assessment of patients, including those with highly complex presentation, using investigative and analytical skills. To formulate individualised management and treatment plans, using clinical reasoning, knowledge of evidence-based practice and utilising a wide range of treatment skills and options.
Responsible for equipment used in carrying out Occupational Therapy duties. To assess, order and fit equipment and to adhere to departmental policy, including competence to use equipment and to ensure the safe use of equipment by others through teaching, training and supervision of practice.
Undertake the measurement and evaluation of your work and current practices using evidence-based practice projects, audit and outcome measures, either individually, with other Neurological Specialists or with other Occupational Therapists and make recommendations for change.

Drivers Licence and car driver.
Qualifications Essential
OT Qualification and NHS experience mandatory. HCPC registered.

Community and Neuro experience is essential.
