Job description Job responsibilities Clinical Responsibilities
Undertake routine nursing procedures and treatments within competence and scope of practice.
Deliver chronic disease management clinics, including but not limited to diabetes, asthma, COPD, hypertension, and cardiovascular disease.
Administer vaccinations and immunisations in accordance with national and local guidelines.
Perform cervical screening (where appropriately trained).
Carry out wound care, dressings, and removal of sutures and clips.
Conduct NHS Health Checks and health promotion activities.
Provide lifestyle advice and support relating to smoking cessation, weight management, healthy living, and disease prevention.
Undertake venepuncture, blood pressure monitoring, ECGs, and other diagnostic procedures as required.
Support infection prevention and control procedures within the practice.
Recognise and respond appropriately to safeguarding concerns.
Maintain accurate and contemporaneous clinical records.
Patient Care
Assess, plan, implement, and evaluate patient care.
Promote patient independence and self-management of long-term conditions.
Provide health education and advice to patients and carers.
Ensure care is delivered in a compassionate, professional, and patient-centred manner.
Professional Responsibilities
Work within the requirements of the Nursing and Midwifery Council (NMC) Code.
Maintain professional registration and continuing professional development.
Participate in clinical supervision, appraisal, and mandatory training.
Contribute to clinical audits, quality improvement initiatives, and service development.
Adhere to practice policies, procedures, and governance requirements.
Team Working
Work collaboratively with GPs, healthcare assistants, pharmacists, social prescribers, and other healthcare professionals.
Participate in practice meetings and multidisciplinary team discussions.
Support colleagues and contribute to a positive working environment.
Administrative Responsibilities
Maintain accurate patient records using the practice clinical system.
Manage referrals and follow-up actions as appropriate.
Ensure clinical stock and equipment are maintained and used safely.
Comply with data protection, confidentiality, and information governance requirements.
Person Specification
Qualifications Essential
Qualifications Registered Nurse with current NMC registration. Evidence of continuing professional development. Experience Experience working as a Registered Nurse. Experience delivering patient-centred care. Experience of immunisations and vaccinations. Experience in health promotion and chronic disease management. Knowledge and Skills Knowledge of primary care nursing services. Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Good IT and clinical record-keeping skills. Understanding of clinical governance, safeguarding, and infection prevention and control. Personal Attributes Professional and compassionate approach. Strong organisational skills. Ability to prioritise workload effectively. Commitment to maintaining high standards of patient care.
Desirable
Previous experience in a GP practice or primary care setting. Cervical screening qualification. Chronic disease management qualifications or experience. Independent Prescriber qualification. Mentorship or teaching experience. Experience using clinical systems such as EMIS Web, SystmOne, or Vision.
