Job summary

Central London Community Healthcare NHS Trust provides a range of Community Services, and the South West Division is recruiting!We have an exciting opportunity for a band 6 Physiotherapist to join our Tooting/Balham Community Therapy Team.The post-holder will have clinical responsibility for assessment and treatment planning for clients referred to the community therapy team. They will be expected to contribute to the development and monitoring of the service. The post holder may be asked to supervise rehab support workers and would be expected to supervise physiotherapy students on practice education placements.The Tooting/Balham Community Therapy Team aims to provide a high quality, responsive rehabilitation service helping adults to regain their independence following illness or injury, to support them with their Long-Term conditions and to promote health and wellbeing.Working collaboratively within a supportive team which comprises of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Rehabilitation Support workers and admin Assistants.As this is a community-based role, with travel between patient homes, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Main duties of the job

To deliver physiotherapy intervention in a variety of home and community settings, as appropriate. Having clinical responsibility for assessment and treatment planning for people referred and will be required to work autonomously and as part of the multidisciplinary care team To work in close collaboration across own and other community teams with referring health and social care professionals and voluntary agencies to optimise patients' journey, care, wellbeing and outcomes. To undertake specialist physiotherapy and holistic assessment of patients for a variety of conditions. As an autonomous practitioner you will interpret results appropriately, use clinical reasoning skills and knowledge of evidence-based practice, in relation to the individual's clinical and psycho-social issues to determine treatment options, recommending the best course of action. Delivering high quality physiotherapy intervention to provide rehabilitation, enablement, facilitation of self-management for a range of patients. To assess, select and recommend equipment which may assist clients in achieving the maximum level of independence and quality of life in daily living activities. This will include ensuring the safe use of the equipment and may include fitting/installing of equipment in client's homes. Providing written and/or verbal reports to referrers and other agencies involved in patients' care and participate in clinical meetings as necessary.

Person Specification

Qualifications Essential

Physiotherapist with appropriate Diploma/Degree Current registration with HCPC Evidence of relevant continuing professional education and training Willingness to undertake further training as appropriate

Desirable

GCSE grade A-C or equivalent in English and Maths Registration with the national regulatory body (when it becomes available)

Experience Essential

Relevant post registration experience covering core rotations including community experience Appreciation of constraints to working in the community Ability to supervise juniors/assistant/students Ability to use electronic clinical system e.g. RIO, EMIS, Cerner Previous NHS experience

Desirable

Experience of providing supervision and delivering training Experience of service development and audit

Skills Essential

Knowledge of PT assessment tools relevant to an adult client group Knowledge of a range of relevant PT interventions relevant to the client group e.g. dementia, Parkinson's, COPD, Falls, etc Knowledge of national policies and procedures relevant to adults An understanding of the principles of clinical governance Self-management skills-managing time and workload Excellent oral and written communication and presentation skills

Flexible about work location - the post holder will be required to travel to other CLCH sites in London
