Job summary
This job is for a Resident Doctor who provides care to inpatients, at Mildmay Hospital UK, at ST1 ST2 level under the supervision of the Medical Director. This involves providing medical care for a complex mix of patients admitted for rehabilitation following illnesses related to HIV infection, with the majority of patients suffering from some degree of neurocognitive impairment, and for medical care to the intermediate care unit for the homeless. The doctor will work as part of a large interdisciplinary team to provide holistic care in order to gain the best outcome for patients health and integration into the community
Main duties of the job
Able to take responsibility for inpatients at Mildmay including daily ward rounds. Other duties include day to day management, administration and patient education and psychological support if applicable.
Able to assess patients with acute problems, liaising with Consultant. When appropriate, referring to emergency services at patients acute centre or the Royal London Hospital. Upon transfer preparing an appropriate letter or report to accompany the patient.

Mildmays hospital team is multi-disciplinary and operates a reactive and fast- moving service. It aims to provide enhanced inpatient care and ensure safe, appropriate and sustainable hospital discharges that meet the needs of individual homeless patients. Through this approach, the team also aims to reduce delayed and premature discharges, and reduce unscheduled admissions, repeat admissions and A&E attendances.

Job description Job responsibilities
Responsibility for inpatients at Mildmay including daily ward rounds. Other duties include day to day management, administration and patient education and psychological support if applicable.
Clerking of new patient admissions, and planning their care whilst at Mildmay in conjunction with nursing staff, and other members of the IDT.
Assessment of patients with acute problems, liaising with Consultant. When appropriate, referring to emergency services at patients acute centre or the Royal London Hospital. Upon transfer preparing an appropriate letter or report to accompany the patient.
Ensuring discharge summaries for all patients are dispatched within 48 hours.
Writing referral letters for patients attending outpatient sessions at other hospitals, whilst at Mildmay.
Liaising with medical staff at other centres e.g. acute centre, as appropriate, to clarify the ongoing management of Mildmay patients.
Providing on-call cover for patients attending Covid care. The level of care for these patients is similar to that of an emergency GP.
Prescribing patients medications on ward charts. Checking patients medications, especially antiretroviral therapies, on admission, and corroborating any unconventional regimens with the Medical Director / their acute centres as appropriate.
Informing the acute centre of relevant changes to their patients whilst at/ upon discharge from Mildmay.
For medications prescribed at Mildmay, checking and signing appropriate forms to request these from either Fairdale Pharmacy or the patients acute centre.
Input to the IDT/ward round meeting with a brief review of each new patient and a short summary for other patients. Contributing to discussion of how medical issues may interact with other aspects of the patients care. Recording in the patients file a note of each IDT/medical ward round.
1. Attending review meetings and discharge planning meetings and preparing written reports summarising the medical issues affecting the patients.
2. At the request of the admissions team, where necessary speaking with referring doctors before a patients admission.
3. Performing relevant venepuncture and chasing results.
