Main duties of the job
Assess, diagnose and treat a varied community caseload, using evidence-based interventions tailored to individual needs. Collaborate across the MDT, offering expert advice and guidance to colleagues, carers and families to support safe, timely care. Demonstrate strong clinical reasoning, communicating clearly and compassionately--especially where there may be barriers to understanding. Contribute to service quality, sharing learning, supporting juniors and students, and helping to drive continuous improvement. Work as a proactive team member, using your initiative and prioritising effectively in a dynamic community setting. You are required to hold a full, valid, UK driving licence and have access to a car to use with business insurance (unless the member of staff has a disability as defined by the Equality Act 2010).

Person Specification
Driving License Essential
You are required to hold a full, valid, UK driving licence and have access to a car to use with business insurance (unless the member of staff has a disability as defined by the Equality Act 2010)
Qualifications & Professional Registration Essential
Degree (or equivalent) in Physiotherapy Registered with HCPC
Desirable
Postgraduate training in community rehabilitation or related area Evidence of specialist CPD
Clinical Assessment & Treatment of Complex Patients Essential
Experience delivering physiotherapy to patients with complex needs/multi-pathologies Ability to undertake advanced assessment and implement evidence-based treatment plans
Caseload Management & Prioritisation Essential
Ability to independently manage and prioritise a demanding caseload
Multidisciplinary Working & Communication Essential
Experience working effectively within MDTs across health and social care
