Job summary
The post-holder will work as an autonomous, accountable advanced practitioner, providing a complete episode of care to patients presenting with undifferentiated and undiagnosed problems. Working within their scope of practice and as an independent prescriber, the ANP will assess, diagnose, treat, prescribe for, refer and signpost patients of all ages across acute, minor illness and long-term condition presentations, both in surgery and during home visits to housebound patients
Main duties of the job
Clinical practice
Provide first-contact clinical assessment for patients presenting with undifferentiated, undiagnosed problems - taking history, examining, investigating, diagnosing and managing the episode of care.
Manage same-day, acute and minor illness presentations across all age groups, including telephone and online triage where required.
Independently prescribe, review and de-prescribe medication within own competence and in line with NICE guidance, the BNF, MHRA safety alerts and the practice formulary
Proactively manage patients with long-term conditions e.g. diabetes, asthma, COPD, hypertension, cardiovascular disease, including structured reviews, treatment optimisation and care planning.
Undertake home visits to housebound and frail patients, exercising sound clinical judgement on what can be safely managed in the community.
Recognise, assess and manage deteriorating, acutely unwell and emergency presentations, escalating appropriately to the duty GP or emergency services.
Request, interpret and act on investigations and results, and arrange onward referral to secondary care and other services as clinically indicated.
Work within own clinical competence, seeking advice from or referring to a GP or appropriate colleague where a presentation falls outside scope of practice.

Hounslow Family Practice is a GP practice serving approximately 6200 patients in Hounslow. We are looking for an experienced and autonomous Advanced Nurse Practitioner to join our multidisciplinary team for two days each week (Mondays and Thursdays).

Maintain registration with the Nursing and Midwifery Council (NMC) and revalidate in line with NMC requirements.
Practise within the NMC Code - legally, safely and ethically - and within agreed scope of practice and protocols.
Maintain and develop own competence through continuing professional development, appraisal and reflective practice.
Support, supervise and act as a clinical resource for practice nurses, HCAs and wider team members as appropriate.
Contribute to the development of practice protocols, patient group directions and clinical pathways.
Work collaboratively with the wider primary care network PCN and community teams.

Qualifications Essential
Registered Nurse with current NMC registration Independent / non-medical prescriber qualification - V300 Master's-level qualification in Advanced Clinical Practice, or equivalent demonstrable Master's-level competence Relevant post-registration clinical qualifications, e.g. minor illness, long-term conditions, physical assessment Evidence of ongoing CPD and NMC revalidation
