Job summary
We are seeking an enthusiastic and motivated Practice Nurse to join our friendly and supportive primary care team. The successful candidate will provide high quality nursing care to our patients population, supporting both acute and long term condition management. You will work as part of a multidisciplinary team delivering evidence based care, health promotion and screening services. The role includes immunisations, cervical screening and supporting patients to manage chronic conditions.
We welcome applications from experienced Practice Nurses with transferable skills who are keen to develop within primary care.

Job description Job responsibilities
To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the Doctors and other members of the primary health care team. The duties will include all tasks normally undertaken by an experienced RGN and in addition any roles agreed between the nurse and the Doctors as appropriate.
Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions.Running well person clinics and health promotion programmes by providing a holistic assessment and lifestyle advice on diet, smoking, alcohol intake and exercise.Assessing problems presented opportunistically by patients, dealing with minor illness.Able to document consultations according to NMC guidelinesEnsures awareness of statutory and local clinical protection procedures, including systems of referral .
