Exciting opportunity to join a large, modern, innovative Practice in Huddersfield (5 minutes from the M62 junction). Looking for an enthusiastic and motivated team player who enjoys working in a successful, high acheiving Practice. Our experienced multidisciplinary team currently consists of 9 GPs, 2 ANPs, 1 PA, 4 Pharmacists, 5 Practice Nurses and 5 HCAs. We use TPP SystmOne.

Main duties of the job
We are seeking experienced prescribing clinical pharmacists to join our GP practice team. Youll work within a supportive, forwardthinking environment that embraces modern IT systems and AIenabled workflows, with a blend of patientfacing and remote clinical activity.
Were looking for pharmacists confident in managing medicinesrelated queries, conducting structured medication reviews, supporting longterm condition management, and contributing to safe, effective prescribing within the multidisciplinary team.
The role will be based on-site in Huddersfield, HD2 2QA

Job description Job responsibilities
Provide clinical medication reviews, including SMRs, for patients with longterm conditions and polypharmacy.
Manage acute and repeat prescription requests, ensuring safe and effective prescribing.
Support longterm condition management and contribute to medicines optimisation across the practice.
Run pharmacistled clinics (face to face and remote) for medicationrelated issues.
Action medicinesrelated tasks from total triage and Patchs online consultations.
Lead on medicines safety, audits, MHRA alerts, and antimicrobial stewardship.
Contribute to QOF, IIF and PCN medicinesrelated objectives.
Provide medicines advice to the multidisciplinary team and support training of learners.
Person Specification
Experience Essential
Experience working in General Practice or a PCN setting. Strong clinical skills in medicines optimisation and longterm condition support.
Qualifications Essential
Registered with the GPhC. Independent Prescriber qualification.
